Job summary

Lower Clapton Group Practice is looking to employ a part-time Clinical Coder and Medical Summariser. The hours of work are 15 hour per week.

The successful candidate will work alongside another Coder/Medical summariser and be responsible for the document management and summarising of inbound medical records and correspondence working as part of a team.

Main duties of the job

The successful candidate will be responsible for the document management and summarising of all inbound medical records and correspondence working as part of a team.

Main Duties include:

Entering electronic and paper correspondence onto the clinical system

Review each document, code and file into patient records or where appropriate send it on to the most appropriate member of either the clinical or administrative teams

Identify all relevant data held within patients paper medical notes and enter the information onto the EMIS Web clinical system

Clarify whether medical information from paper records is relevant to either the patients current condition or their past medical history

Updating and maintaining patients electronic medical summaries

This role would ideally suit someone who enjoys being able to manage their own workload, whilst also functioning well as part of a wider team. A willingness to learn new skills and improve on existing ones will be essential in this new role.

The Practice is open from 8.00am to 8.00pm. The successful applicant will be a member of the Administration team completing patient related administrative tasks. Experience of working in a general practice is an advantage. Knowledge of EMIS Web computer system is essential.

About us

We are a large but close team of the usual staff but also innovative PCN roles such as a vocational OT, mental health worker as well as a fantastic team of clinical pharmacists and FCPs as well as a great wellbeing team. We are a 7 partner practice one of which is our lead nurse and another is our experienced ANP.

This role is currently based at the Lower Clapton Group Practice. We are about to move intoan amazing new building attached to a very special building (Google Portico Hackney) so there is a lot of excitement in the practice.

Job responsibilities

As part of an administration team the Clinical Coder / Summariser is responsible for:-

Scanning, coding and filing information into patient records from documents coming into the practice electronically or in paper form.

This is a key role with the aim of reducing the paperwork of the practices GPs using a workflow process overseen by a GP Partner to ensure robust clinical governance.

Organising and monitoring the summarising of patient medical records to meet Practice targets ensuring every patient has a complete, accurate and well organised electronic medical record

Main Duties include:

Entering electronic and paper correspondence onto the clinical system

Reviewing each document, coding and filing them into patient records or where appropriate sending it on to the most appropriate member of either the clinical or administrative teams

Identify all relevant data held within patients paper medical notes and enter the information onto the EMIS Web clinical system

Responsible for following Practice procedures to summarise patient notes (of new patients and existing patients as directed by a clinician) in a timely manner, resulting in an accurate electronic patient record

Clarify whether medical information from paper records is relevant to either the patients current condition or their past medical history

Updating and maintaining patients electronic medical summaries

Processing information within the practices clinical workflow system

Responsible for checking electronic records received via GP to GP transfer are checked against the relevant paper record to ensure all information has been extracted and accurately coded

Responsible for the management of coding in relation to any new outbreaks of infection of any nature

Actively raise any quality issues with the Workflow Supervisor in a timely manner

Ensuring the quality of clinical coding carried meets Practice standards and raising/reporting any issues in a timely manner

Responsible for dealing with queries relating to coding

Participate in audits as required

Undertake Reception Duties as and when required

Answering Telephone Calls as required
