Job summary

We are looking for a proactive, compassionate Practice Nurse to join our Nursing team. You will work collaboratively with HCAs, Phlebotomists, Nurse Practitioners, Registrars, Salaried GPs, GP Partners, and our well-established, supportive administration team.

This part-time role is for 16 hours across two fixed working days per week - Wednesday, including extended access, and Thursday.

Our practice is situated in a charming market town, featuring a picturesque town park with views of the River Severn, an ideal spot for a relaxing walk or lunch break!

Main duties of the job

  • To undertake clinical management of patients in accordance with practice protocols, policies, patient group directives and evidence-based practice
  • To undertake health needs assessment of patients in the Practice both on an individual and group basis
  • To deliver health education, health screening and health promotion as both opportunistic and planned interventions
  • Accurate and thorough data collection and recording
  • Experience in complex wound care
  • To ensure all patients, visitors and colleagues are treated with respect, dignity always in a friendly and professional way using clear verbal and written communication
  • Continual personal development in knowledge and skill to ensure the needs of the Practice population are met
  • Extended Access at a premium rate of pay

About us

We are a team made up of 9 GP Partners (male and female) 6 Salaried GPs (male and female), 2 Nurse Practitioners, 4 Nurses, 5 HCAs and 4 Phlebotomists, working across two rural sites. We are a training practice with several Registrars and Medical Students.

We have a large, knowledgeable administration team, and an excellent team spirit throughout.

The Team is dedicated to providing an excellent service to our patients whilst promoting healthy living.

We are part of a Primary Care Network consisting of 9 practices. We meet regularly and support each other clinically and non-clinically.

Job description

Job responsibilities

Manage caseload of patients on the clinical system across sites

Management of patients including monitoring (BP, ECG) and vaccination programmes

Support the team in dealing with clinical emergencies

Support the teams with safeguarding matters

Knowledge of chronic diseases (asthma, COPD, diabetes, HF, etc.)

Good working knowledge and experience incomplex dressings and supporting pathways

Opportunistic health promotion (diet, smoking, alcohol, exercise)

Ensure Nurse Registration, revalidation and indemnity are up to date

Input accurate and timely information into patients EHR using approved coding

Work safely within IPC, undertaking audits, aseptic techniques when required

Initiate and participate in clinical audits

Actively participate in significant even analysis (SEA)

Maintain appropriate training, skills and knowledge

Contribute towards achieving audit targets for the Practice

Participate in management of recording of medical supplies and stock expiry dates

Maintain testing and recording of treatment room equipment, reporting any malfunctions to management

Chaperoning of patients and clinicians as required

Adhere to all relevant policies including IPC and cold chain management

Use of PPE and laundering provided uniform as per guidance

Apply spill control and decontamination policies

Demonstrate good practice for equal opportunities, culture and diversity

Any other appropriate duties as requested by Management or Partners
