Job summary

Are you passionate about all things digital?

We are looking for a highly motivated and enthusiastic individual to support the delivery of ongoing improvement to digital services, ultimately resulting in improved patient outcomes as well as more efficient administrative and clinical processes.

You will have experience in business / service improvement, and expertise in change management to lead our digital transformation activities

A thorough understanding of general practice systems and processes is essential.

Key Responsibilities

The core objective of the Digital & Transformation Lead role is to support the delivery of ongoing improvement to administrative, patient and clinical services. The role will require experience in business / service improvement, and an expertise in change management. A thorough understanding of general practice systems and processes is essential.

About us

This new and exciting role will help support our new Primary Care Network (PCN), Bognor Coastal Alliance (BCA).

BCA consists of a network of 3 practices, with a combined 36000 patients that will be working together within a geographical area with the aim of becoming the most respected Primary Care provider in the UK.

Job description

Job responsibilities

Main duties include:

Lead an improvement approach to change including building on quality improvement within the practices, PCN and system wide approaches to problem solving.

Improve adoption and/or optimisation of new technology to enhance patient access and experience and increase Practice and PCN productivity.

Build relationships and facilitate collaboration between practices and the wider system, to support delivery of care to patients

Review and improve the PCNs digital maturity.

Support population health management and health inequality

Understanding of type and intensity of support/training needs in practices and the PCN and coordinate this support.

Facilitate clinically led innovation and the effective adoption of improvement initiatives, including integrated working across practices and level to improve access to services for patients.

Develop new and manage an overall transformation plan for the PCN, including identifying interdependencies, managing risks, considering potential impacts on the wider organisation.

Work with the PCN executive team and member practices to improve data quality, understand demand, capacity and activity and drive improvement.

Support interoperability between a variety of digital solutions to foster collaborative working and improve the efficiency of processes.

Support screening programmes across the PCN

Support practice submission of toolkit

Write DPIAs as and when required

Liaise with the DPO

Project Management

Deliver significant projects to support the PCN and lead to improvements to health outcomes for the patient population.

Manage entire lifecycle of a programme of system change.

Manage the set up and delivery of multiple projects within a programme of work, working with colleagues and subject matter experts to scope, plan, design and deliver those projects.

Review project reports and risks logs, advising project leads on escalation processes and support delivery of projects where there may be significant barriers to delivery.

Interrogate and analyse sometimes highly complex information to support the delivery of clear project reporting and project outcomes.

Write and submit high level project reports for the PCN board as required, showing progress against milestones, status, resource requirements, issues, risk and dependencies, making recommendations where a range of options may exist.

Develop key performance indicators and milestones for specific programmes and projects, managing conflicts and ensuring that interdependencies are managed effectively.

Support organisational change and the uptake of initiatives that support excellence.

Demonstrate flexibility and support new programmes and projects where necessary.

Service Improvements

Responsible for the delivery of robust information systems to ensure that all project documentation is stored and used effectively (dashboards, highlight reports, risks and issues logs).

Support practices to modify recall systems for the purpose of QoF in order to make use of the best clinical skill mix and maximise outcomes for each patient contact

Develop programme and project management tools, reports and templates that are transferable between projects, and can be used for the whole project lifetime, from design to delivery.

Improve accuracy of Practice registers and increase income by improving prevalence and coding

Be a champion for best practice, learning from experience and from others, supporting the spread of innovation within the PCN.

Maintain a good understanding of PCN contracts and service specifications and communicate condensed and informative versions to practices to reduce duplication of work

Maintain expert and up-to-date knowledge in the use of clinical computer system reporting modules and automation features. Design and run clinical system searches on request. Design, run, analyse and report on patient surveys as required.

Communication

Communicate, engage and build relationships with key strategic policy makers to influence the development of strategy and policies.

Engage and manage relationships with stakeholders to achieve a common purpose and realise mutual benefits.

Forge close relationships across the PCN to facilitate an effective approach to delivery of projects and programmes of work

Ensure PCN members understand QOF/DES/IIF etc and their role in achievement and provide general, role-specific and individual training and feedback to ensure delivery
