Job summary

Northgate Surgery are looking for an Advanced Nurse Practitioner/Advanced Clinical Practioner to join our team. We are looking for 20-37.5 hours per week. Highly competitive salary. Must be able to work Mondays.

We are a friendly, advanced training practice with 4 partners, 7 salaried GPs, GP trainees, 5 Advanced Nurse/clinical practitioners, medical students, pharmacists and an excellent nursing and administrative team.

Main duties of the job

Work with within our current ACP team dealing with acute and routine patient problems which will consist of face to face, telephone, online consultations and home visit appointments.

Efficiently manage the recalls and reviews of patients with chronic diseases

Receive patients in clinic. Assessment, diagnosis, treatment, prescribing, referral as appropriate. Providing public health, health protection and promotion programmes that improve health and reduce inequalities

Homes visits to patients who are appropriate to be seen by a Advanced Clinical/Nurse Practitioner- in their own homes or in a care home setting. Carry out health reviews, medication reviews, care planning. Assessment/diagnosis/referrral.

Ensure that significant events are reported using Practice procedures

We have a fantastic opportunity for an enthusiastic Advanced Nurse/clinical Practitioner to join our team. Northgate Surgery is a friendly and supportive working environment. The Practice has a patient population of 15,400 and is still growing. The Practice is based in the centre of Pontefract. The Practice provides parking.

We are a training practice and we play an active role in our local PCN and have excellent clinical, admin and management support teams to support the right individual in the role.

Job description

Job responsibilities

Work with the GPs/Practice Manager/ACP team to run the appointment system, advising on the best methods to obtain the maximum benefit to our patients.

Telephone triage patients with undifferentiated and undiagnosed problems and make an assessment of their healthcare needs. Booking patients in to your own clinics.

Work with within our current ACP team dealing with acute and routine patient problems which will consist of face to face, telephone and home visit appointments.

To maintain own 3 yearly registration with the NMC

To adhere to the NMC Code of Professional Conduct

To maintain a safe environment for all Primary Health Care staff, patients, carers and co-workers from other agencies. Ensure that all statutory regulations (e.g. Health & Safety, COSHH, etc) are adhered to

To represent the ACP team in a professional manner by attire and attitude to patients and staff both in and out of the Practice

To maintain own continuous professional development and ensure PDP kept up-to-date in line with the requirements of the Practice

Attend and participate in staff meetings

Any other reasonable duties that may be required from time-to-time
