Job summary

We are looking to recruit a Care Homes Care Co-Ordinator to join our Multi-Disciplinary Team at Abbey House Medical Practice.

The focus of this role is to work closely with GPs and other Primary Care professionals within the Practice and PCN to identify and manage a caseload of identified patients and to make sure that appropriate support is made available to these patients and their carers to ensure that their changing needs are addressed.

This is a permanent position - minimum of 25 hours per week but up to full time hours available; Monday to Friday.

Main duties of the job

We are seeking an individual who is self-motivated and enthusiastic, approachable, compassionate, organised and must be able to work as part of a team. Previous experience of working in a people-centered environment is essential.

The successful candidate must demonstrate good communication skills face to face, over the telephone and written down plus be able to actively listen to both patients and colleagues.

A good knowledge of computers is essential. Previous use of SystmOne would be an advantage however full training will be given.

Hourly rate depending on experience.

Job description

Job responsibilities

DUTIES TO INCLUDE

1. Utilise population health intelligence to proactively identify and work with a cohort of patients, which includes but not limited to the elderly, at risk individuals, care/nursing home residents, those eligible for vaccination, parents/carers of children eligible for vaccinations, to deliver personalised care.

2. Take a leading role in arranging vaccination clinics; to include preparation for clinics, supporting the nursing teams with stock management, booking patients/children in for vaccination clinics, appropriate signposting and accurate documentation.

3. Take on the Care Home Champion role for the practice, acting as the key point of communication and link between the care homes and the rest of the team in the practice and liaising with outside agencies where necessary. Oversee the administration of prescription requests, ward rounds, visits, registrations and deaths, and other queries.

4. Provide coordination and navigation for people and their carers, across health and care services, working closely with the care co-ordinator (safeguarding), other care co-ordinators, social prescribing link workers, and other primary care professionals.

5. Support patients to utilise decision aids in preparation for a shared decision-making conversation and to understand their level of knowledge, skills and confidence when engaging with their health and wellbeing.

6. Holistically bring together all of a persons identified care and support needs and explore their options to meet these into a single personalised care and support plan, in line with best practice.

7. Help patients to manage their needs through answering queries, making and managing appointments, and ensuring that people have good quality written or verbal information to help them make choices about their care.

8. Assist people to access self-management education courses, peer support or interventions that support them in their health and wellbeing and increase their activation level, as well as signposting to access appropriate benefits, training and education opportunities.

9. Where appropriate refer people back to other health professionals.

10. Support the coordination and delivery of MDTs.

11. To work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures whilst managing own workload to deliver the practice priorities.

12. Raise awareness within the practice of shared-decision making and decision support tools

13. Raise awareness of how to identify patients who may benefit from shared decision making

14. Ensure that all activities are recorded, monitored and evaluated.

15. Attend all meetings as required.

16. Work in accordance with practice policies, ensuring that all work and procedures are undertaken in accordance with issued guidelines.

17. Attend any training courses and supervision sessions as required.

18. Promote the professional image and services offered by the surgery.

19. Any other duties that may be required and agreed by the Doctors and Management Team
