Job summary

We are looking to recruit a Safeguarding Care Co-Ordinator to join our Multi-Disciplinary Team at Abbey House Medical Practice.

The focus of this role is to work closely with GPs and other Primary Care professionals within the Practice and PCN to identify and manage a caseload of identified patients and to make sure that appropriate support is made available to these patients and their carers to ensure that their changing needs are addressed.

This is a permanent position - minimum of 25 hours per week but up to full time hours available; Monday to Friday.

Main duties of the job

We are seeking an individual who is self-motivated and enthusiastic, approachable, compassionate, organised and must be able to work as part of a team. Previous experience of working in a people-centered environment is essential.

The successful candidate must demonstrate good communication skills face to face, over the telephone and written down plus be able to actively listen to both patients and colleagues.

A good knowledge of computers is essential. Previous use of SystmOne would be an advantage however full training will be given.

Hourly rate depending on experience.

Job responsibilities

DUTIES TO INCLUDE:

1. Proactively identify and work with a cohort of people; bringing together all of a persons identified care and support needs, exploring their options to meet these into a single personalised care and support plan, in line with best practice and using the available decision support aids.

2. Help people to manage their needs, answering their queries and supporting them to make appointments, referring them back to other health professionals or members of the MDT where appropriate.

3. Work with safeguarding professionals, to assist completion of safeguarding forms and case conference paperwork, including MASH referrals and child protection reports.

4. Taking an administrative lead with safeguarding for adults and children; processing correspondence from external agencies, maintaining safeguarding registers, managing requests for information.

5. Organising the practice safeguarding policies and keeping the practice intranet up to date in this respect, as well as supporting training.

6. Take charge of the annual safeguarding audit.

7. Take an administrative lead in PREVENT policies and queries in the practice.

8. Maintain an up to date log of contacts and services within the safeguarding area and establish and maintain good communication links with external contacts.

9. Provide coordination and navigation for people and their carers across health and care services, alongside working closely with the care coordinators, social prescribing link workers and other primary care roles.

10. Support the coordination, delivery and attendance at safeguarding & MDT meetings.

11. Ensure that all activities are recorded accurately, monitored and evaluated.

12. Attend all meetings as required.

13. Work in accordance with practice policies, ensuring that all work and procedures are undertaken in accordance with issued guidelines.

14. Attend any training courses and supervision sessions as required.

15. Promote the professional image and services offered by the surgery.

16. Any other duties that may be required and agreed by the Doctors and Management Team
