Job summary

An exciting opportunity has arisen for a Social Prescribing Link Worker to join our friendly team at Abbey House Medical Practice.

This new role will involve assisting our patients to take control of their health and wellbeing taking a holistic approach, connecting people to community groups and other services for both practical and emotional support. This is particularly important for people with long-term health conditions, those who are lonely or isolated or those with complex social needs which impact on wellbeing.

This is a permanent position and the hours are negotiable - a minimum of 25 hours up to a maximum 37.5 hours per week.

Main duties of the job

We are seeking an individual who is enthusiastic, compassionate, approachable, organised & must be able to work as part of a team. The successful candidate must demonstrate good communication skills in order to provide timely and accurate verbal and written communication to patients, carers and a wide variety of healthcare professionals.

IT experience is essential (MS Office & Internet) and a knowledge of SystmOne would be an advantage however full training will be given.

Previous experience of working in a "people-centered environment" is preferred and knowledge of the local area is desirable, along with an awareness of the challenges faced in the community.

The successful candidate must have or must be willing to complete Safeguarding Certification, training as set out by the Personalised Care Institute and the NHSE/I online learning programme. The practice is committed in supporting employees in their continued professional development.

We are offering this post as a permanent position and the hours are negotiable up to a maximum of 37.5 hours per week.

Hourly rate £11 per hour.

RESPONSIBLE TO: MDT Team Leader

RESPONSIBLE FOR: Empowering patients to take control of their health and wellbeing taking a holistic approach, connecting people to community groups and other services for practical and emotional support. To help strengthen community and personal resilience and reduce health inequalities, particularly for people with long term conditions, are lonely or isolated, or have complex social needs which impact on wellbeing.

DUTIES TO INCLUDE

1. Take referrals from primary care colleagues and other agencies to support the health and wellbeing of patients.

2. Provide personalised support to individuals, their families and carers to take control of their wellbeing, live independently and improve their health outcomes.

3. Develop trusting relationships by giving people time and focus on what matters to them.

4. Take a holistic approach, based on the persons priorities, and the wider determinants of health.

5. Co-produce a simple personalised care and support plan to improve health and wellbeing, introducing or reconnecting people to community groups and statutory services; signposting where appropriate and it matters to the person.

6. Evaluate how far the actions in the care and support plan are meeting the patients health and wellbeing needs.

7. Explore and support access to a personal health budget where appropriate.

8. Manage and prioritise your own caseload, in accordance with the needs, priorities and any urgent support required by individuals on the caseload.

9. Where required and as appropriate, refer people back to other health professionals within the network.

10. Ensure those organisations and groups that are supported, have basic safeguarding processes for vulnerable individuals and can provide opportunities for the person to develop friendships, a sense of belonging, and build knowledge, skills and confidence.

11. Work together with all local partners to collectively ensure that local VCSE organisations and community groups are sustainable and that community assets are nurtured, by making them aware of small grants or micro-commissioning if available, including providing support to set up new community groups and services, where gaps are identified in local provision.

12. Attend the peer support networks run by NHS England/Improvement.

13. Have a role in educating staff and GPs on what other services are available within the community and how/when patients can access them.

14. Ensure that all activities are recorded, monitored and evaluated.

15. Attend all meetings as required.

16. Work in accordance with practice policies, ensuring that all work and procedures are undertaken in accordance with issued guidelines.

17. Attend any training courses and supervision sessions as required.

18. Promote the professional image and services offered by the surgery.

19. Any other duties that may be required and agreed by the Doctors and Management Team
