An exciting opportunity has arisen for an experienced
registered nurse to join our established nursing team at The Greenway Medical
Practice.
The ideal candidate will have experience of working in a
primary care environment, managing a range of chronic health conditions, and be
able to fulfil the expectations set out in the job description and person
specification. However, we are open to applications from nurses of varying
experience with a willingness to undertake further training to progress.
You will join a team of friendly, supportive and
forward-thinking clinicians, and like minded nurses and healthcare assistants,
who strive to provide the best outcomes for their patients.
The role is full time, but we would welcome applicants
wanting part time hours or a job share.

Main duties of the job
As Practice
Nurse at The Greenway Medical Practice, you will be responsible for the
implementation of processes for the effective management of patients with
long-term conditions using evidence-based practice including care for elderly
and housebound patients.
Working as a senior member of the practice
multidisciplinary team, the post holder will ensure nursing services are
delivered effectively to the entitled patient population.
The Practice Nurse will be responsible for a number
of clinical areas such asinfection prevention and
control,health promotion, chronic disease management,
health promotion, well women and well man clinics, as well as actively
supporting the practice management team in the reviewing and delivery of
clinical policy and procedure.
About us
The practice serves almost 9,500 patients in Cleckheaton and
its surrounding areas. We were last rated GOOD by CQC in May 2016, and received
outstanding results in the 2024 GP Patient Survey
We are an active member of the Spen Health and Wellbeing
Primary Care Network, working closely with our partners.
The team is led by 3 GP partners, an ANP partner and the
practice manager, and employs a salaried GP, an experienced ANP and an Advanced
Clinical Practitioner. The clinical team are supported by 2 part time practice
nurses, 2 part time healthcare assistants, and an office manager leading a team
of experienced receptionists, administrators and secretaries.
The practice is also benefits from being a long-standing
training practice, accommodating a GP registrar in training each year.
The practice has the benefit of accessing ARRS roles such as
first contact physiotherapists, pharmacists, social prescribing link workers, and
mental health practitioner, and has a dedicated GP Assistant working in
practice 3 days a week.
Occupying a substantial part of the first floor in a
purpose-built health centre, the practice also benefits from free on-site
parking, and is a short walk from the amenities of Cleckheaton.

The following are the core responsibilities of the Practice
Nurse. There may be, on occasion, a requirement to carry out other tasks. This
will be dependent upon factors such as workload and staffing levels:
- To act as or support the Infection Prevention Control (IPC)
lead and to work to the IPC Policy providing or supporting the implementation
of audits, training, support and evidence of compliance to the practice manager
- To produce or support the production of the IPC annual
statement in conjunction with the practice manager
- To develop, implement and embed an effective care management
programme for the frail and housebound patients registered at the practice
- To assess the needs of patients ensuring the provision of
healthcare is appropriate, incorporating evidence-based practice
- To develop, implement and embed health promotion and
wellbeing programmes
- To implement and evaluate individual treatment plans for
chronic disease patients that promote health and wellbeing
- To identify, manage and support patients at risk of
developing long-term conditions, preventing adverse effects to patients health
- As required, to provide routine nursing care to patients in
accordance with clinical based evidence, NICE and the NSF
