*Part Time (up to 30 hours)

We are a growing, evolving, busy Southeast London training practice with a population of just under 20,000 patients.

We operate and deliver our service over 3 sites. The post holder will be expected to work across all sites. Driving and owning a car is an advantage.

We are a large multidisciplinary team, inclusive of GP partners, salaried GPs, trainee GPs, Advanced Practitioner, Pharmacists, Physician Associate, Physios, Mental Health worker, dietician, Practice nurses and Phlebotomy/ Health care assistants. We have a large reception and admin team.

We are planning to expand our nursing team and looking for a competent Practice Nurse to join our friendly supportive team.

The successful candidate must have previous experience of Practice Nursing and demonstrate an understanding of general practice.

Ideally you will have qualifications and be able to demonstrate competencies in areas of Long term condition management, childhood and baby immunisations, vaccinations, travel, wound care and cervical screening.

Support with access to ongoing role relevant training will be considered.

Hours: Negotiable

Main duties of the job

To be responsible for the delivery of practice nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population.

The practice nurse will be responsible for a number of clinical areas such as health promotion, chronic disease management, health prevention, well women and well man clinics, as well as supporting the management team in the reviewing of clinical policy and procedure.

  • Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects on the patient’s health
  • Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
  • Provide vaccination and travel medicine services
  • Provide wound care (ulcer /Doppler etc.) to patients
  • Request pathology services as necessary
  • Process pathology results as required
  • Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary / specialist care as required
  • Maintain accurate clinical records in conjunction with extant legislation
  • Ensure read codes are used effectively
  • Maintain chronic disease registers
  • Develop, implement, and embed well woman clinics
  • Develop, implement, and embed well man clinics
  • Implement vaccination programmes for adults and children
  • Prioritise health issues and intervene appropriately
