Job Summary
To be responsible for the delivery of evidence-based nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population. Working autonomously, the Practice Nurse will be responsible for a number of clinical areas such as health promotion, chronic disease management, health prevention, well women and well man clinics, as well as supporting the management team in the reviewing of clinical policy and procedure.

Primary Responsibilities
The following are the core responsibilities of the nurse prescriber. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:
a. Work effectively as a member of the Nursing Team- sharing responsibilities and support.
b. Continually review clinical practices, responding to national policies and initiatives where appropriate
c. Maintain professional registration and adhere to revalidation requirements
d. Develop, implement and embed health promotion and well-being programmes
e. Develop, implement and evaluate individual treatment plans for acute, non-acute conditions
f. Support the clinical team in the development of individual treatment plans for chronic disease patients especially those who have respiratory conditions.
g. Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health
h. Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
i. Prescribe medication in accordance with PGD and within own scope of practice
j. Review medication for effectiveness in accordance with patient need and national guidance
k. Provide information and advice on prescribed or over-the-counter medication on side effects, common interactions and dosage
l. Support patients to ensure compliance with prescribed medications
m. Provide wound care (ulcer /Doppler etc.) to patients
n. Provide travel medicine services
o. Request pathology services as necessary, processing and interpreting results as required
p. Refer patients directly to other services as appropriate
q. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary / specialist care as required
r. Maintain accurate clinical records in conjunction with extant legislation
s. Ensure read codes are used effectively
t. Maintain chronic disease registers
u. Develop, implement and embed well woman clinics
v. Develop, implement and embed well man clinics
w. Chaperone patients where necessary
x. Assist GPs with minor surgery when required
y. Prioritise health issues and intervene appropriately
z. Support the team in dealing with clinical emergencies
aa. Recognise, assess and refer appropriately patients presenting with mental health needs
bb. Recognise, assess and refer patients with community health needs, developing strategies for effective care pathways
cc. Implement vaccination programmes for adults and children
dd. Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
ee. Support the clinical team with all safeguarding matters, in accordance with local and national policies
ff. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
Secondary Responsibilities
In addition to the primary responsibilities, the nurse prescriber may be requested to:
a. Act as the audit lead, effectively utilising the audit cycle
b. Monitor and ensure the safe storage, rotation and disposal of medicaments
c. Contribute to the implementation of an effective training programme for staff and students within the practice
d. Provide clinical supervision for the nursing team
e. Participate in local initiatives to enhance service delivery and patient care
f. Support and participate in shared learning within the practice
g. Be aware of and contribute to the planning and implementation of CCG patient pathway initiatives
h. Participate in the review of significant and near-miss events applying a structured approach i.e. root cause analysis (RCA).
