Job summary
This is an exciting opportunity for a newly qualified salaried
GP role to experience and deliver a wide variety of primary care clinical work.
We are looking for GPs to work within the Enhanced Access hub, with the
opportunity of working in a local GP practice and care homes in Southwark
depending on service needs.
This combines dealing mainly with acute, on-the-day problems at Enhanced Access, chronic disease management of complex elderly patients in care homes, as well as routine GP work at New Mill Street Surgery.
The development session is an opportunity to further an individual interest, develop a clinical service or a leadership role that supports the development of both the individual and QHS services.

Main duties of the job
- In accordance with the QHS rota, as agreed, the post-holder will make themselves available to undertake a variety of duties including face to face consultations, telephone consultations and queries, visiting patients at home, prescribing and dealing with queries, paperwork and correspondence in a timely fashion
- Assessing the health needs of patients with undifferentiated and undiagnosed problems
- Screening patients for disease risk factors and early signs of illness
- Contributing to QOF / enhanced service requirements and recording data appropriately
- In consultation with patients and in line with current practice disease management protocols, developing care plans for health related illnesses
- Appropriately managing the appointment list and referring to other care providers as appropriate within the QHS protocols
- Recording clear and contemporaneous consultation notes to agreed standards
- Prescribing in accordance with the accepted prescribing formulary (or generically) and the ICB QIPP program whenever this is clinically appropriate
- Proactively supporting the EA Clinical Lead or Practice Manager in day to day delivery of primary care services, for example: coordination of the EA operational service delivery on a day to day basis; governance and risk management, CPD and keeping clinical protocols up to date

blocks for at scale service delivery and wider system integration through
neighbourhoods that are the delivery units of North Southwark Primary Care
Network (NSPCN) and our integrated Borough based system. These bring together local health and care
organisations (including social care, the voluntary sector and patient groups)
to work collaboratively to deliver more joined up, patient-centred services.

the North Southwark population seven days a week. This service was initially designed to
provide additional capacity for member practices catering acute presentations,
however over the recent years, we expanded to incorporate some routine primary
care services to support local needs.

QHS, has been appointed by North Southwark PCN to
deliver Enhanced Access Service to
the North Southwark population seven days a week. This service was initially designed to
provide additional capacity for member practices catering acute presentations,
however over the recent years, we expanded to incorporate some routine primary
care services to support local needs.
QHS GP Care Home
Service provides primary medical care for residents of all
older adults care homes across Southwark. There are seven care homes which all
receive dedicated weekly visits for GP ward rounds ensuring continuity of care.
The QHS GP Care Home Service team also includes a dedicated clinical
pharmacist, nurse extensivist, nurse associates, dementia specialist nurses and
a care coordinator. There are regular MDT meetings with the care homes, local
geriatricians, social workers, mental health team and dieticians that enable us
to deliver truly holistic, long-term, multidisciplinary care not seen in most
primary care settings.
New Mill Street Surgery is a small, but dynamic and forward thinking GP practice of 7,300
patients in the heart of Shad Thames. As it is run by Quay Health Solutions,
New Mill Street Surgery is at the forefront of testing new ways of working in
the borough and hosts a variety of clinicians (including GP's, ANP's, Nursing
Associates, FCP Physios, Clinical Pharmacists and Mental Health Practitioners)
resulting in a well-rounded and diverse clinical team.
QHS is also working
with NHS Southwark ICS on a programme of development to support transformation
of local health services. We are working
collaboratively with providers and patients to co-produce solutions to the
challenges we face and maximise the impact of our services on quality and
outcomes of patient care.
