Chineham Medical Practice Practice Nurse
Chineham Medical Practice is looking for a highly motivated registered nurse to join our existing Practice nursing team.
Job-type : Part-time, Permanent
Salary: Dependant on experience
Hours: 24 per week split over 3 days
Main duties of the job
The post holder will work as part of the primary healthcare
team providing Personal Medical Services to the patients of the Practice,
including treatment room care and chronic disease management.
Assist in
and perform routine tasks related to patient care as directed by senior nursing
staff and GPs
To assess
the needs of patients ensuring the provision of healthcare is appropriate,
incorporating evidence-based practice
To identify,
manage and support patients at risk of developing long-term conditions,
preventing adverse effects to patients health
To request
pathology services and process pathology results as required
Chronic
disease management: Diabetes, COPD, Asthma and Contraception
To ensure
compliance with the practice chaperone policy
To assist
GPs with minor surgery when required
To
prioritise health issues and intervene appropriately
To support
the team in dealing with clinical emergencies
To provide a
range of nursing services, including cytology, wound care, doppler, travel
services, CHD/stroke monitoring, baby clinics, vaccinations and immunisations,
health checks, suture removal, assist GPs with minor surgeries and coil
fittings etc.
To ensure
read codes are used effectively
Comply with
QOF, enhanced services and medicine management initiatives to maintain high
standards
Any other
delegated duties appropriate to the post
