Job summary

We are looking to recruit two full time, skilled, experienced and enthusiastic Pharmacy Technicians to join our Primary Care Network and to work with our Pharmacists.

You will work under the supervision of the Practice Clinical Pharmacist to ensure the safe, accurate and timely supply of prescribed medication.

You will work with the administration and reception teams to deal with enquiries from patients, pharmacists and other external clinicians.

Main duties of the job

You will work under the supervision of the Practice Clinical Pharmacist to ensure the safe, accurate and timely supply of prescribed medication.

Provide technical and administrative support to the Clinical Pharmacist and Clinicians.

Support the Practice Teams with queries around medication from patients.

Complete audits around patient safety in the prescribing of medication and present the information for sharing with the clinical team.

Preston Park Community PCN is made up of five local surgeries who have been working together for a number of years. We have approximately 57,000 patients across the PCN and Stanford Medical Centre is the lead practice for the PCN. This role may involve working across more than one practice. We are all high achieving QoF practices and offer all the National and Local Commissioned Services. Job responsibilities

Administrative

To support, train and oversee the administrative staff involved in the preparation of repeat prescriptions and to participate in the daily preparation of repeat prescriptions in order to ensure high quality, safe and timely repeat prescribing.

To support the administration staff with medicines reconciliation in accordance with practice protocols and to update patients medical records / prescribing accordingly.

To support the achievement of the practices prescribing Quality and Outcomes Framework targets.

To contribute to the review and development of repeat prescribing protocols /Standard Operating Procedures to improve the efficiency and effectiveness of repeat prescribing systems.

To assist in the appropriate monitoring and management of prescribing the budgets and to liaise with the medicines management team at the CCG.

To prepare evidence based resources and information to support the implementation of rational cost-effective prescribing.

To take a proactive approach to the safe handling of prescriptions, including assessment of risk and making recommendations for improvement.

To assist in the submission of claims for personally administered drugs as required.

Clinical

To respond to medication queries from patients and staff in a professional manner referring to the appropriate GP in accordance with practice protocols.

To advise on the sourcing and safe management of medicines as appropriate.

To participate in medication audits.

To provide advice to GPs, staff and patients in changes to prescribing in order to support the improvement of prescribing safety, quality and cost effectiveness.

To support the Practice Based Pharmacist to provide medication reviews, patient information leaflets, medicine awareness.

Supporting medication reviews and medicines reconciliation for new care home patients and synchronising medicines for patient transfers between care settings, linking with local community pharmacists, and referring to the pharmacist for structured medication reviews.

Other

Developing relationships with other pharmacy technicians, pharmacists and members of the multi-disciplinary team to support integration of the pharmacy team across health and social care including primary care, community pharmacy, secondary care and mental health.
