Job summary

We are looking to recruit two friendly and enthusiastic full time Care Coordinators to join our Primary Care Network. The post holders will play a key role in actively managing the administrative processes of patient pathways and to ensure a smooth and efficient service for patients throughout their journey. The roles will also provide an effective administration service to the multidisciplinary team.

Being a Care Coordinator is demanding role and will be best suited for someone who works well under pressure and is able to manage their time and work load effectively. The role encompasses administrative tasks, being the first point of contact for patients, and working with clinicians to provide safe and effective patient care. If you want to be at the forefront of patient care, in a friendly and enthusiastic team, then this is the role for you.

Main duties of the job

This is an interesting and varied role involving desk based and customer facing work, so you will need excellent proven Customer Service skills and have a confident and friendly personality. Good IT skills are also required.

Ideal candidates will promote an efficient and high quality service to all internal and external clients inclusive of patients, families, carers, clinicians and members of the public.

About us

Preston Park Community PCN is made up of five local surgeries who have been working together for a number of years. We have approximately 57,000 patients across the PCN and Stanford Medical Centre is the lead practice for the PCN. This role will be working across more than one practice. We are all high achieving QoF practices and offer all the National and Local Commissioned Services. We work very well together and have the aims to innovate and improve the services we offer our patients. Works closely with the GPs and other primary care professionals within the PCN to identify and manage a caseload of identified patients, making sure that appropriate support is made available to them and their carers, and ensuring that their changing needs are addressed.

Plays an integral role in facilitating the care home MDT, to improve the continuity of care by acting as a point of contact for residents, families and professionals.

Contributes to tackling inequalities in health and social care. An ethos of promotion of independence, shared decision making, personalisation and partnership working is integral to this post. As the role evolves the care coordinator will undertake direct work with patients and families to develop personalised care plans.

  • NVQ 3 or equivalent and/or relevant basic/first level professional qualification

Experience

Essential

  • Minimum of 1 year of experience of working with healthcare professionals and or previous experience in the NHS or social care or relevant field
  • Able to prioritise and manage own workload
  • Able to work as part of a team
  • Excellent interpersonal skills
  • Excellent organisational and administration skills
  • Experience of coordinating and liaising with multiple stakeholders or individuals to meet specified outcomes
  • Experience handling confidential/sensitive information
  • Experience of providing advice/signposting to service users
  • Knowledge of Microsoft Office suite

  • Experience of coproduction with patients or service-users
  • Experience of using technology and digital tools to support health and wellbeing
  • Knowledge of Information Governance and data quality
