We are a forward thinking and growing Primary Care Network (PCN) consisting of 3 GP practices, Allerton and Westfield, St Martins Medical Centre and Woodhouse Medical Practice, with a population size of approximately 27,000 patients. We are seeking an enthusiastic individual to join our established team to support the effective running of the PCN.

Main duties of the job

The role of PCN Project/Care Coordinator/Administrator is an essential part of our PCN team which supports the provision of the full range of NHS Primary Care Services. You will be working alongside our friendly and patient focused clinical and management teams in each of the 3 practices, providing an essential administrative support function and promoting the delivery of the best care to all our patients. You must work with autonomy and a high degree of confidentiality, sensitivity, discretion, empathy and tact.

Working closely and under supervision of our PCN Manager in accordance with specific PCN and practice guidelines and protocols, the post-holder will be responsible for providing general assistance, administration (including meeting administration) and operational support; creating, running and analysing report data from clinical systems and within MS Office applications (Word, Excel and Outlook); creating and managing small projects, defined to deliver desired outcomes in accordance with population health data for our patients.

PCN Operations Care Coordinator

Reports To :

Role Description :

Chapeltown Primary Care Network (PCN) consists of 3 GP practices, Allerton and Westfield, St Martins Medical Centre and Woodhouse Medical Practice, with a population size of approximately 27,000 patients. The Care Coordinator role is to support the effective running of the PCN alongside the PCN Manager, whilst projecting a positive and supportive image.

Primary Responsibilities :

Provide general assistance, administration and operational support to the PCN Manager and Clinical Director.

Create, run and analyse report data from clinical systems (S1 and EMIS) and within MS Office applications (Word, Excel and Outlook) and Ardens.

Meeting administration - calendar invites, agendas, presentation packs, compilation and distribution of meeting minutes and ensure actions completed in a timely fashion.

Ensure monthly report packs are accurate and up to date to support PCN operation, exec and board meetings.

Create and manage small projects, defined by or in conjunction with the PCN Manager, to deliver desired outcomes in accordance with population health data for our patients.

- Covid and Flu Vaccination Programme

- Cancer Screening

- NHS Health Checks

Management of intranet (Teamnet) and PCN website

Management of appointment rotas.

Secondary Responsibilities :

Day to day patient interaction both in practice and in patients own homes.

Follow, support and comply with practice procedures, IT, infrastructure and telecom systems, and work in accordance with written protocols and procedures.

Contribute toward the team ethic and support patients in line with the PCN principles and values.

Use own judgement, resourcefulness, common sense and local knowledge, to respond to enquiries and requests.

Strong organisation skills.

Any other tasks allocated by PCN Manager and Clinical Director.
