Job summary

We have a fantastic opportunity to join the team at East Park Medical Centre as a Care Navigator - a crucial role in Primary Care, in supporting our patients with accessing the appropriate services for their needs.

You will be based at East Park Medical Centre, or our annex site Halton Medical Practice to cover shifts.

The role is PART TIME with a view of working 2 days (15 hours) to 3 days (22.5 hours) per week.

The successful candidate will be expected to work on a Wednesday and Friday as minimum, covering shift patterns from 07:45-18:30 - I would be happy to explore longer days (condensed hours) providing that Wednesdays and Fridays are covered.

Main duties of the job

Main Duties (but not limited to):

- Inbound and outbound contact with patients, visitors and service providers (phone, email and face to face)

- Effectively signposting patient requests to the suitable / appropriate service/s eg GP / Nurse appointment, community pharmacy, extended-access service etc

- Responsible for the appointment booking of patients, and effective appointment management

- Supporting our clinical colleagues with requests throughout the day eg patient contact, appointment management

- Managing documents; incoming and outgoing mail, and patient notes, Subject Access Requests, and private work requests

- Ensuring professionalism, confidentiality and privacy is maintained at all times

- Advocating respect, dignity and patience at all times

Job responsibilities

Main Purpose of Job:

To provide a focal point of contact for patients of the practice and administrative support to the practices clinicians.

Main Duties:

Welcoming patients and visitors face to face and over telephone

Ensure confidentiality is maintained at all times

Establish reason behind visitors attendance/call to surgery in a friendly and courteous manner

Ensure patient is dealt with in an efficient and appropriate way

Ensure enquiries/telephone calls are actioned in a prompt manner and where necessary passed to the appropriate responsible person

Management of Patient Registration:

Take necessary details from patient to ensure appropriate registration form is provided to patient

Ensure all required details on the registration forms are complete

Register patient on computer and book new patient check with Practice nurse or Health Care assistant

Appointment management:

Locate appropriate appointment type based on information provided by patient with appropriate healthcare professional

Ensure necessary details are obtained from patient to book appointment i.e. Name and address, reason for attendance Confirm the correct patient has been booked in to the appointment slot

Provide an efficient appointment booking service

Effectively handling requests for cancellation and rearranging of appointments

Actioning requests/directions from doctors/nurses diary management

Create requests for home visits and passing clinically based messages promptly

Ensure all necessary, appropriate and relevant information is obtained to deal with problems

Manage daily results and distribute to clinicians

Calling Patients for checks

React to directions to call in patient groups where required and with appropriate training

Repeat prescribing and online access management

Administration of outgoing communications and / or samples

Manage NHS E mail system

Safeguard all keys, cash and accessible patient identifiable information

Review personal and teams responsibilities regularly

Use initiative and competence to identify outstanding actions and tasks accordingly

Action relevant tasks and responsibilities promptly, efficiently and professionally adhering to Practice protocols and staff handbook

Report inadequacies in building and be responsible for Health and Safety

Identify own training needs

Participate in Significant event analysis process

Accept and react to change

Work reliably, calmly and competently under pressure

Be positive, honest and professional in your approach

Communicate capably, act resourcefully

Work at all times in an organised way and in accordance with protocols, procedures and regulation including the staff handbook
