Job summary

We are a well organized, friendly, innovative PMS practice operating from a purpose built surgery in the centre of Oldham. We are a well established practice with a CQC rating of 'good' and high QOF achievement

Main duties of the job

To provide a full range of medical services as defined in the core PMS Contract.

To provide 6 clinical sessions per week pro rata, one of these sessions will include covering late calls and repeat prescriptions.

To respond to medical problems presented by patients including history taking, examination, diagnosis, investigation, treatment and referral where appropriate.

Provide health promotion and preventative health care and advice where appropriate.

Provide emergency cover for colleagues.

Maintain patients clinical records.

To participate in and support the movement towards fully computerised and electronic methods of data collection and record management whilst ensuring appropriate communication with all professionals.

Responsible for promoting and safeguarding the welfare of the patients you are responsible for or come into contact with.

About us

Hopwood House is a purpose built centre to meet the modern requirements of both patients and staff and is equipped to give the medical team every assistance in the care of the patient.

We are a busy Practice with approximately 6000 patients situated half a mile from Oldham Town Centre. Oldham is a friendly lively town with sport, the arts and entertainment taking a high profile. On the doorstep is the Pennine moorland encompassing a number of delightful villages but only 15 minutes away from the vibrant city centre of Manchester it has good schools and motorway links.

We have a Principal GP, 1 Salaried GP, 1 ANP, 4 Practice Nurses and a very experienced HCA. We also have a Clinical Pharmacist a First Contact Physiotherapist, Safeguarding Care Co-ordinator and a Focused Care worker. We are a training practice and currently have an ST3 and ST2 trainee at the practice.

Job responsibilities

HOPWOOD HOUSE

Working within a multi-disciplinary team serving 6000 patients, the post involves the delivery of medical services to patients registered with the Practice. There will also be involvement in the development, monitoring and evaluation of the Practice.

The hours are by flexible arrangement with the postholder to meet the operational needs of the Practice. The post is for 6 sessions a week.

This post is to provide a full range of high quality primary care services in line with approved protocols and evidence based medicine to the Practice population.

The post holder will support the Practice in meeting the targets and objectives outlined in the NSFs and other nationally, locally and Practice objectives agreed with the CCG.

Clinical:

To provide a full range of medical services as defined in the core PMS Contract.

To provide 6 clinical sessions per week pro rata, one of these sessions will include covering late calls and repeat prescriptions.

Participate in the production of regular performance monitoring reports as required.

Personal Development:

The post holder(s) will be offered protected time for personal and professional development and encouraged to define and address their personal development needs, which may include developing skills in a GP Specialist role.

The post holder(s) will be offered support in the form of mentorship and peer review and will be expected to utilise this support in developing the service they can offer.
