Job summary

At Hereford Medical Group (HMG) we are currently looking to hear from any experienced paramedics to join our home visiting team, who are supported by a dedicated team of allied health professionals and administrative staff.

Main duties of the job

The successful candidate will be completing home visits to assess patients presenting with acute illness or chronic disease conditions. They will be clinically examining and assessing patent needs from a physiological and psychological perspective, while diagnosing, planning, implementing and evaluating interventions or treatments for patients with complex needs.

The aim to enhance patient care, to reduce emergency calls and/or admissions to hospital and to enhance the organisation of care to reduce unnecessary procedures and to support care home staff to have greater autonomy within their roles and gain an understanding of the role of primary care services
