Job responsibilities
This role requires consolidation of
specialist knowledge and skills in general practice nursing demonstrating a
depth of knowledge, understanding and competence that supports evidenced
informed, complex, autonomous and independent decision-making and care in
general practice and related settings. This role will require personal
resilience, management, clinical leadership and supervision and mentorship of
others in the general practice nursing team and providing an effective learning
environment for staff and students in the wider team. The role will require an
innovative approach in supporting and developing new models and strategies for
service delivery, usually incorporating inter-professional and inter-agency
approaches to monitor and improve care. General Practice Nurses deliver care to
the practice population, but also need to have an understanding of the public
health profile and population needs in order to be proactive in ensuring
services are, as far as possible, matched to need. The post
holder will, at all times, deliver the high level of evidence based individual
care to the Practice population. The post holder have the responsibility for
the provision of nursing services related to General Practice which will
include participating in the assessment of patient care to include treatment,
preventative care, screening and patient education. The post holder will work
at times without supervision.
Job
Responsibilities:
The post holder will be able to practice a range of clinical
skills developed through experience and training according to Practice
protocol.
The post holder will ensure clinical practice is safe and
effective and remains within boundaries of competence and to acknowledge
limitations.
Providing

Assisting
with Minor Procedures (when required)
Chaperoning
Pathological
Specimens and Investigative Procedures
Undertake
the collection of pathological specimens including intravenous blood samples,
swabs etc.
Perform
investigative procedures requested by GPs
Administrative
and Professional Responsibilities
Ensure
confidentiality at all times
Ensure
accurate and legible notes of all consultations and treatments are recorded in
patients notes
Ensure
the clinical computer system is kept up to date with accurate details recorded
Attend
and participate in Practice meetings as and when required
Report
any problems to the Nurse Team Leader/Practice Manager
Chronic
Disease Clinic Responsibilities
The post holder will be involved in the maintenance of chronic
disease registers.
The post holder will evaluate the outcome of care, making changes
to care plans for individual patients and modifying their own practice with the
patient.
The post holder will provide management/health education for
Diabetes, Asthma, COPD and CHD.
Administrative
and Professional Responsibilities
Ensure
the clinical computer system is kept up to date with accurate details recorded
Ensure
recall dates are entered onto the clinical system
Liaise
with other members of the Primary Health Care Team and other agencies in order
to ensure that appropriate care is given
Promote
health promotion
The
post holder will participate in new methods of work using evidence based
research to maintain high standards of care
The
post holder will participate in standard writing, audit and monitoring to
develop and implement action plans resulting from audit when needed
Any
ad-hoc duties required by GP partners
Training
and Personal Development
Training
requirements will be monitored by annual appraisals and will be in accordance
with Practice requirements and developments
The
post holder will complete annual statutory/mandatory training
The
post holder will develop and maintain an in-house training programme reporting
to the Nurse Team Leader
Personal
Development plans will be encouraged and supported by the Practice. It is the
individuals responsibility to remain up to date with recent developments
Liaison
As
well as the nursing team there is a need to work closely with reception and
admin to ensure the smooth running of the Practice
Any
problems should be reported initially to the Nurse Team Leader
There
is also the need to establish and maintain good liaison with other surgeries
and agencies including secondary care
Confidentiality
The
post holder may have access to confidential information relating to patients
and their carers, Practice staff and other healthcare workers; they may also
have access to information relating to the Practice such as a business
organisation. All such information from any source should be regarded as
strictly confidential.
Information
relating to patients, carers, colleagues and other healthcare workers may only
be divulged to authorised persons in accordance with the Practice policies and
procedures relating to confidentiality and the protection of personal and
sensitive data
The
