Fenham Hall Medical Group, NE4 9XD This is an exciting opportunity for an Advanced Nurse Practitioner currently working in Primary Care or a Advanced Clinical practitioner colleague working in another sector looking for a new challenge to develop or enhance their minor illness and ailments skills. Salary is competitive and negotiable upon experience but will be in the range of AFC bands 7 to 8a. This is a busy role, caring for our diverse patient population, working alongside our on call GP and current ANP. The role will alternate between pre-booked same day face to face appointments, and telephone triage and booking into your own appointments. We are looking for an enthusiastic, flexible and hardworking individual who will thrive on the challenges of working in tight knit supportive Primary Care team. The successful applicant will work autonomously but supported by our friendly and experienced clinical and admin teams. We encourage the development of our practice team, and currently teach years 1,3 and 5 Medical Students and the successful applicant will have the opportunity to be involved with this. We aspire to be a GP training practice by August 2023. An independent prescribing qualification would be an advantage but we would be happy to support the right person to develop and achieve this. Ideally this would be a fulltime role, Monday to Friday, but we would consider a job share arrangement.

We are looking to recruit to replace our current ANP who specialises in the care of patients presenting with acute problems. This will predominantly be face to face, in pre booked same day appointments. We have a young patient population so the sessions will include a diverse range of patients including babies and young children. The successful applicant will work closely with the GP and nursing teams and a named GP for mentoring and personal development. An on call GP is available at all times for support and advice

Supported by 2 Practice Nurses and 2 HCAs. Our clinicians are supported by a strong and experienced admin and reception team. Refer patients directly to other services and agencies as appropriate. Provision of a holistic service to patients and their families, developing where appropriate an on-going plan or care and support, with an emphasis on prevention and self-care. Work with the Practice team to ensure that National Service Frameworks are being delivered. Contribute to the practices achievement of its quality targets to maintain the high standards of patient care and service delivery. Contribute to the development and set up of new patient services and participate in initiatives to improve existing patient services.
