Main duties of the job

The successful candidate will work closely with our Advanced Clinical Pharmacist and will potentially have a patient-facing role, working directly with patients to promote safe and effective use of medication and self-care. The role will involve working as part of a multidisciplinary team to develop and run processes for medicines management on transfer of care and systems for safer prescribing.

Brunel 3 PCN has a population of 30,000+ patients. We are looking for an additional Pharmacist to join our Pharmacy Team comprising of Pharmacists, Technicians and Prescription Clerks. You can help us develop medicines optimisation services within our Primary Care Network. The team is led by an Advanced Clinical Pharmacist and the team is based together at Crossroads Surgery.

Job responsibilities

The post holder is a pharmacist who works alongside and supports the general practice teams within their professional boundaries. The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice. The post holder would be assigned a surgery but also provide cover when needed across the PCN.

Support and mentorship will be offered by the PCN practices to enable the Pharmacist to develop in their role.

KEY ASPECTS OF THE ROLE:

Work as part of a multi-disciplinary primary healthcare team in both a back office and potential patient-facing role.

Provide pro-active support for effective medicines management for patients suffering from chronic diseases.

Within the limits of professional competency manage medication for patients with common minor ailments.

Undertake clinical medication reviews to proactively manage patients with complex polypharmacy.

Provide primary support to practice staff regarding prescription and medication queries.

Support the practices repeat prescribing systems and deal with acute prescription requests and medicines reconciliation on transfer of care.

Implement systems for safer prescribing, providing expertise in clinical medicines advice.

Provide clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services.

Ensure that the practices integrate with community and hospital pharmacies to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload.
