Job summary

Are you passionate about providing exceptional patient care? Do you want to work with a team of GPs and Nurses who share these values?

We are looking for a Practice Nurse to join our large teaching practice, working across Home Ground Surgery and our branch surgery Crossroads. You will work autonomously with the help and guidance of a team of supportive Nurse Practitioners and GPs providing a range of treatment room duties. Experience in the care of long term conditions; diabetes and/or asthma would be favourable.

We support and encourage the professional development of our Nursing team to help us provide the best care to our patients. We are also a friendly bunch and have regular social events throughout the year.

Job responsibilities

The role of Practice Nurse is to provide a high standard of nursing care for patients across the practice population. The post holder will undertake a range of nursing assessments and provide appropriate care and treatment in conjunction with GPs and other healthcare colleagues, working in accordance with practice policies and protocols. The post holder will strive to improve standards of care and be involved in identifying practice population needs.

Chronic Disease management is an essential part of the role, and the post holder will be expected to become familiar with, and help the practice to meet, targets relating to elements of the GP GMS contract, for example the Quality and Outcomes Framework and Local and National Enhanced Services. The post holder will also offer mentoring and support to colleagues including Healthcare Assistants and Administrators.
