Job summary

An exciting opportunity has arisen for an enthusiastic, motivated and experienced Nurse to join our large and friendly multidisciplinary team in fantastic purpose built premises in Garstang.

Main duties of the job

We are looking for a qualified and experiencednurse to manage our nursing services and support our extensive team of nurses and HCAs. Qualifications and experience in the management of Diabetes and Respiratory would be an advantage and other primary care nursing skills including; Management of Hypertension, CKD, CHD; vaccination/immunisation (including childhood), wound care and cervical cytology would also be desirable. We want friendly and personable candidates who have the communication skills, enthusiasm and aptitude to work alongside our existing experienced nursing team. JOB DESCRIPTION

SERVICE OVERSIGHT AND DEVELOPMENT

To monitor all nursing services provided by the practice to ensure most efficient use of resources and service provision to patients. This will include supporting regular review and implementation of working rotas to ensure an appropriate level of the various nursing services are being provided. This will entail reviewing and liaising with other service providers such as extended hours and urgent care provision. Liaising with managers and clinical colleagues to ensure adequate service provision to meet all aspects of the Quality and Outcomes Framework. Develop and implement services and pathways to improve health in individuals and groups.

NURSING TEAM SUPPORT

Support all members of the nursing team to work effectively together to provide a wide range of nursing services. This will include undertaking audit and setting and monitoring standards of care. Work with colleagues to identify personal development and training needs and provide support and mentoring where needed. Meet regularly with the various nursing teams and co-ordinate and organise whole team nursing meetings as needed. Liaise with other practices within the WREN network to help facilitate shared pathways, development and training.

TRAINING

Co-ordinate and enable training and mentoring for all members of the nursing team. Oversee all aspects of practice nursing staff training and external student training; liaising with all members of the clinical team as appropriate to facilitate shadowing and learning. Assist management and admin with the co-ordination of clinical rotas to facilitate this. Source and promote external training placements within the practice. Liaise with the Training Hub and external training providers to facilitate appropriate student placements following approval from the GP training leads.

APPRAISALS

Carry out annual appraisals for all the nursing team and ensure any actions or training identified are adhered to in a timely fashion in liaison with the Business Manager.

RECRUITMENT

Assist in the recruitment of members of the nursing team, including assisting with the production of job descriptions, adverts and with the interview process.

STUDENTS

Responsible for overseeing the provision of training placements for nursing students as agreed with the partners. Providing students with a comprehensive timetable during their placement with the practice and acting as their mentor. This may also include training of nurse associates and paramedic students.

INFECTION CONTROL

Practice lead for infection control. Responsible for regular audit and ensuring staff and the practice are compliant in all aspects. Completion of the annual infection control audit and reporting this to the Quality Services Manager.

VACCINATION LEAD

Practice lead for vaccination campaigns; providing pro-active management to ensure high compliance, particularly amongst vulnerable groups of patients. Ensuring all national vaccination programmes are delivered appropriately.

QOF and QUALITY CONTRACT

Working with the partners and managers to ensure all KPIs are met and supporting staff to ensure understanding and compliance in accurately recording patient data and completing appropriate practice templates. Monitoring performance against the KPIs and implementing remedial strategies if required.

EXTENDED ACCESS

Communicate and liaise with the Extended Access service to improve the services to the practice and network and work with the practice team to ensure utilisation of the service for our patients.

CHRONIC DISEASE MANAGEMENT

Undertake clinical reviews in line with practice protocols for patients with chronic disease including respiratory and diabetes.

GENERAL TREATMENT ROOM DUTIES

To provide cover as necessary to undertake all aspects of treatment room duties including wound care, venepuncture, vaccinations.

PRACTICE POLICIES

Ensuring all relevant nursing policies are up to date and available on Teamnet for all the team to access along with all relevant PGDs. Ensuring all fridge temperatures are recorded on Teamnet as per practice policy.

Any other duties deemed appropriate and delegated to the role by the partners as new developments occur.
