Job summary

Abertawe Medical Partnership requires an enthusiastic, highly motivated Nurse to work within a busy but friendly practice. Experience of working in a primary care setting is essential. We are a dedicated team delivering a high standard of patient care. The surgery is supported by an excellent clinical and administrative team. The Practice Nurse will be able to assess the nursing needs of patients. She will be expected to provide appropriate care/treatment in conjunction with the GP, or independently, as appropriate and according to Practice policy, protocol etc. The Practice Nurse will advise patients in general health care and minor ailments with referral to General Practitioners when necessary. It is important that all members of staff are prepared to undertake additional duties or relinquish existing duties in order to maintain the efficient running of the Practice.

Main duties of the job

Main Duties and Responsibilities

Venepuncture. Testing pathology samples e.g. blood, urine. Blood pressure and weight management. Aid/chaperone to GP during clinical procedures, e.g. minor operations, vasectomy, etc. Specimen collection e.g. msu, pregnancy tests, occult blood, faeces for O&S, sputum, etc. The Nurse will be responsible to putting labels on samples, form filling and ensuring timely preparation of specimens for collection to laboratory. General observations e.g. BP, pulse and temperature. ECG nurse responsible for recharging ecg machine. Use and care of nebulisers. Ensure the maintenance of stock levels in the Treatment Room relating to patient care. The Nurse will administer childhood immunisations in conjunction with the GP as per the child immunisation programme. Administration of travel vaccination and travel advice. Flu vaccination clinics/any other vaccination clinics as deemed appropriate.

Large MDT comprising of 6 partner practice, 2 salaried Drs, 4 part time PNs, Cluster Pharmacist, Cluster Paramedic with additional support from the Mental Health team and the Community Outreach Mental Health Homeless Nurse. Based on two sites.

Dressings simple and complex.

Health promotion and chronic disease management.

Chronic disease management procedures may include:

Identify and target patients for chronic disease clinics as per GMS contract requirements.

Organise and facilitate Nurse Led Chronic Disease Management Clinics i.e. Well-Person, Well-Woman, Diabetes, Asthma & COPD, Heart Disease prevention, Epilepsy, Smoking Cessation, alcohol advice, weight/diet control or any other which may be considered appropriate.

Liaison with other agencies as required.

Teaching patients self-care where appropriate.

Procedures may include:

Checking demographic details.

Taking a full medical, family and lifestyle history.

Recording previous screening procedures and immunisation status.

Giving immunisation and vaccinations when indicated.

Urine testing, blood pressure, height and weight measurement.

Teaching breast self-examination and encouraging breast awareness.

Cervical smears

Testing visual acuity.

Peak flow/spirometry measurement, and inhaler technique.

Teaching relaxation techniques.

Liaising with administrative staff to ensure that claim forms are completed and general office procedures maintained.

Providing where appropriate patient education and information.

Recording of relevant data onto Practice clinical computer system.
