Job summary

We are seeking a pharmacist educated to degree level, registered with the General Pharmaceutical Council (GPhC), possessing a Masters Degree (MPharm) or equivalent.

You will need to be flexible in your approach to work as the role will be working across two sites; providing pharmacy services, including;

Optimise the use of medicines

Support the practice with regards to prescription and medication queries

Improve the safety and quality of prescribing

Providing support and education to patients on self-management and ensure referral to appropriate healthcare professionals if required

Implement and manage the repeat prescribing process for practice and patient requests.

This is an exciting opportunity in which you will gain experience and make a real impact. This role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within the practice.

Previous experience of working in Primary Care is not essential, training will be given, and you will be supported by our in-house Pharmacy Team and The Doctors

Main duties of the job

Work as part of a multi-disciplinary team.

Take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy

Provide primary support to general practice staff with regards to prescription and medication queries.

Support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient in the practice

Support the practice integrate with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare, and help manage workload. The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice.
