Job summary

The main purpose of this role is to provide proactive clinical support to vulnerable patients and those with complex physical or mental health needs who are considered to be at high risk of future hospital admission within the Winchester and Andover Primary Care areas. The aim is to improve their quality of life and healthcare experience and therefore reduce the risk of emergency hospital admission.

We are looking for an exceptional nurse to join our team at Mid Hampshire Healthcare which covers the Winchester City area.

Key Tasks:

· Conduct initial home assessment visits to housebound patients with a view to identifying needs and supporting the patient and family/carers to facilitate the patient remaining in their own home safely and with the correct support clinically and socially.

· Review patients holistically to identify or monitor patients with complex healthcare needs, social isolation, risk of falls, mobility issues, diet and mental health problems. To improve their quality of life and prevent avoidable crisis.

· Delegate patients to other clinical team members to undertake reviews or provide continued support and work collaboratively in the development of a care pathway.

· Carry out blood tests, flu vaccinations, injections, take urine samples and mobile ECG readings as required to support patient.

· Work with the PCT Care Coordinator to review patients over the age of 70 with post discharge visits to prevent unplanned re-admissions as far as possible.

· Liaise with GPs to make management decisions regarding patients care and future planning. Supported by community geriatrician as appropriate.

· Liaise with Community Teams and In Reach Team to facilitate early and smoother discharge of patients out of hospital.

· Use EMIS Template to record all consultations/interventions with patients. Update as appropriate.

· Introduce patients to voluntary and NHS services as appropriate, assist patients to access an assessment from Adult Services Social Care and Support to source care information if required.

· Share information and work collaboratively with GPs, Practice Nurses, Community Care Teams, Social Services, Older Peoples Mental Health Team and voluntary Agencies, providing consent has been given, to deliver the best outcome for the patient.

· Attend Virtual Ward and Whiteboard meetings when necessary.

· Support clinical students on placement with the team.

· Attend joint visits and support the learning and development of clinical colleagues.
