Care Navigator/admin The Village Medical Centre (Woolton)

The Village Medical Centre is looking to recruit a care navigator for 25 hours per week (to be worked over 4 days) to join our small established friendly practice team. Customer service and attention to detail skills are essential, as is the ability to learn and solve problems. Ideal candidate will be computer literate and be able to multi task within a busy environment. There will be a set rota, but flexibility across shift patterns between 8.00am and 6.30pm will be required.Applications welcomed from those with either general practice or other relevant experience.

We are a small general practice based in Woolton, voted as one of the best places to live! We have 3600 patients currently on our list. We have 3 GPs, 1 practice nurse and 1 HCA who has a dual role as care navigator. We have an established team who have worked here for many years. We are members of Swagga PCN and have some clinics provided by them at our surgery.

The ideal candidate must be a team player who is confident in: - Answering the telephone, dealing with patients F2F in reception. - Making appts, dealing with patient queries. repeat prescriptions, new patient registrations, address changes, temporary resident registrations, patient referrals, etc. - Using AccurX messaging. - Using Choose & Book if required. - Signposting patients to local services. scanning, clinical coding, workflow, etc). - Other ad hoc administrative and clerical tasks. The duties and responsibilities to be undertaken by members of the practice administration team may include any or all of the items in the following list. Duties may be varied from time to time under the direction of the senior administrator/practice manager, dependent on current and evolving practice workload and staffing levels:

  • Opening up of practice premises and maintaining security in accordance with Practice protocols
  • Maintaining and monitoring the practice appointments system
  • Processing personal and telephone requests for appointments, visits and telephone consultations and ensuring callers are directed to the appropriate healthcare professional
  • Processing and distributing incoming (and outgoing) mail
  • Taking messages and passing on information
  • Filing and retrieving paperwork
  • Processing repeat prescriptions in accordance with practice guidelines
  • Computer data entry/data allocation and collation; processing and recording information in accordance with practice procedures
  • Initiating contact with and responding to requests from patients, other team member and associated healthcare agencies and providers
  • Clearing and re-stock consulting rooms as required
  • Providing clerical assistance to practice staff as required and scanning
  • Ordering, re-ordering and monitoring of stationery if required and other supplies

  • Keeping the reception area, notice-boards and leaflet dispensers tidy and free from obstructions and clutter

  • Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks
  • Making effective use of training to update knowledge and skills
  • Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards
  • Actively reporting of health and safety hazards and infection hazards immediately when recognised
  • Keeping own work areas and general / patient areas generally clean, assisting in the maintenance of general standards of cleanliness consistent with the scope of the job holders role
  • Undertaking periodic infection control training (minimum annually)
