Job summary
Job Purpose: To work within the Multidisciplinary Team (MDT), to provide Burton Latimer Medical Centre with a social prescribing service that takes a holistic approach, has a focus on what matters to me and connects patients to community based services and support to enable them to maintain their health and wellbeing, support behaviour change to achieve a healthy lifestyle, and which addresses health inequalities
Main duties of the job
1. To work with and support individuals to identify their needs, develop personalised care and support plans, build up their resilience, and increase their active involvement with their local communities.
2. To work with and support community groups to be accessible and sustainable and help people start new community groups, working collaboratively with all local partners.
