Job summary

We have an exciting opportunity for a Primary Care Network Integration Manager to join our PCN.

We are looking for an individual who shares our values and has the energy, skills and passion for delivering a smooth patient journey.

Main duties of the job

The post holder will act as the main point of contact to coordinate the successful implementation of the frailty Multi-Disciplinary Teams (MDTs) approach and support collaborative working between multiple organisations to achieve improved outcomes for the local population.

The post holder will be responsible for operational planning and design, implementation and review of the new service model in consultation with the PCN Manager, the PCN Executive, the Clinical Frailty Lead, iCAN, and staff from a wide range of health (primary and secondary), social care and voluntary sector provider organisations.

The post holder will be responsible for supporting the roll out of the iCAN (integrated Care Across Northamptonshire) local priorities for Health and Social Care Integration across the PCN footprint.

Main Duties & Responsibilities

The post holder will:

· Develop close working relationships with the PCN Clinical Frailty Lead, Clinical and Non Clinical members of the PCN practices and various partner health, social care and voluntary sector organisations.

· Organise and project manage the Frailty MDT scheme, to ensure success designing, implementing and embedding a MDT across the PCN footprint, ensuring plans are credible, viable and achievable.

· Co-ordinate a multi-disciplinary team, some of whom may be employed by other organisations, to deliver the outcomes of the service.

· Work with the iCAN steering group to design and deliver a service model that meets the demands of the population.

· Develop a process for defining and coordinating cases for discussion at the Frailty MDT, including standard operating procedures and triage systems, organise suitable clinic sessions and review processes having oversight of actions and onward referrals.

· Where required ensure input of wider specialist health or care professionals to the MDT to ensure best outcome for the patient.

· Provide regular feedback and updates to the PCN Manager, PCN Executive and the iCAN steering group.

Specific Role Requirements

a. Be the first point of contact for Practice Managers, Clinical Directors and external health and social care colleagues with regards to delivery of this project;

b. Help develop joint working and relationships across networks and its stakeholders;

c. Maintain relationships across all core member Practices;

d. Build external relationships and identify opportunities to work with wider system partners;

e. Engage with a wide range of appropriate stakeholders to encourage participation in integrated team working through a MDT approach.

f. Be responsible for the development of operational processes of the PCN Frailty model and for the development of Standard Operating Procedures;

g. Analyse service modelling, develop performance indicators and data collection requirements to monitor the service;

h. Collate feedback / analysis data on behalf of the PCN to report to the CCG and other organisations as required;

i. Carry out any other duties as may reasonably be required by the Clinical Frailty Lead, Clinical Director, PCN Board and its members.

Communication

The post holder will provide and receive highly complex, highly sensitive or highly contentious information. The post holder will be skilled in areas of persuasion, motivation, training and empathy and reassurance. These skills will be used:

a. to ensure co-operation and agreement from others or to break down barriers in understanding;

b. to present complex, sensitive or contentious information to a large group of staff members or members of the public;

c. to provide and receive complex, sensitive or contentious information, where there are significant barriers to acceptance which need to be overcome using developed interpersonal and communication skills such as would be required when communicating in a hostile, antagonistic or highly emotive atmosphere.

  • Good knowledge of NHS policy particularly understanding of the NHS long-term plan and the GP Contract Agreement.

Desirable

  • Formal Management Qualification or equivalent significant experience
  • Hold a project management qualification appropriate to the role
  • Understanding of the structure of social care.
  • Knowledge of QOF and Enhanced Services.
  • Understand the payment structures within primary care.

Experience

Essential

  • Experience of working on clinical systems.

Desirable

  • Minimum of 12 months experience in a senior role working with/in Primary Care.
  • Experience of developing and implementing common systems and processes across teams and across service boundaries.
  • Experience of facilitating and managing change.
  • Experience in managing records and data protection issues.
