Job summary
We are recruiting an experienced and enthusiastic Pharmacy Technician to join our medicines team and work closely with our Clinical Pharmacist. You will be responsible for the delivery of medicines optimisation, management change programmes and activities in General Practice to reduce medication waste, increase cost effective prescribing and increase patient safety.
Main duties of the job
Working as part of our multidisciplinary team supporting the practice pharmacist to ensure the effective and efficient use of medicines.
To help patients get the best from their medicines in agreement with approved protocols, improve repeat prescribing processes, promote the use of repeat dispensing, minimise clinical risk and reduce medicines waste.
To support the pharmacist by preparing and delivering performance reports/briefings for the practice.
Contributing to the development and implementation of patient medication changes and supporting technical medication reviews.
Providing advice and support to patients regarding improved medicines compliance and assisting in the management of high-risk drug monitoring.

Pemberton Surgery is part of Wigan Central PCN and a well established general practice with a patient population of approx 10,300 patients.
We are situated in a purpose built health centre which is shared by 2 GP Practices, community clinic, dentist and Pharmacy.
We have a large practice team comprising of 4 GP Partners, A salaried GP, 2 ANP's,3 (currently 2) practice nurses, HCA, Clinical Pharmacist, Practice
manager, Operations manager, Administration coordinator, Reception supervisor,
Secretary and 11 admin/receptionists. In addition to the core staff we work with
Additional roles staff (ARRS) who currently comprise of Community link worker,
Care -coordinator, Clinical pharmacist, physiotherapists and Mental Health
practitioner.

Job responsibilities
To support the practice pharmacist to ensure that processes are in place for the effective dissemination of prescribing related policies, guidelines and alerts e.g. MHRA safety alerts, GMMMG guidance.
To support the identification and sharing of best practice and ensure these are reflected in working practices and plans.
To support the pharmacist in completing prescribing audits and delivering performance reports/briefings for the practice.
Be responsible for a high standard of work supporting the delivery of agreed projects, ensuring inputs are on time, to quality standards in a cost effective manner.
Demonstrate effective stakeholder engagement across a multidisciplinary team and organisations.
To complete and keep updated, any mandatory training requirements required associated with the role.
To contribute to the development of and to implement patient change programmes within the GP practice and associated care homes to improve quality in prescribing.
To implement agreed medicines support programmes/services in line with practice priorities and identified patient needs.
To work with the practice pharmacist and support completion of medication reviews; this may include domiciliary visits to review concordance and implement measures to optimise medicines use and identify/reduce waste.
Provide verbal and written advice, information and support to patients to support medicines compliance and medicines waste reduction and refer to practice pharmacist as required.
To investigate, identify and advise the practice on additional prescribing actions to ensure prescribing spend is within budget.
To manage and deal with multiple complex problem solving issues involving all aspects of medicines management and quality concurrently, using own knowledge and skills within a framework of broad policies and guidance and being aware of own limitations and referring them to the practice pharmacist when appropriate.
To assist the management of high risk drug monitoring and patients with shared care agreements.
To participate in the identification and development of and be responsible for implementation of practice medicines management and quality policies and guidelines within practice, care homes and social care in line with national and local policies and compliance with medicines related acts to improve cost effective and safe prescribing.
To share relevant medicines related communication with practice staff as appropriate.
To ensure appropriate systems are in place for medicines reconciliation during transfers of care.
To support the practice in the implementation of medicines managfement and optimisation projects to deliver cost savings as part of the QIPP programme.
