Job summary
We have a fantastic opportunity for a new Social Prescriber to join our expanding
health and wellbeing team for a fixed term of 12 months.
We are looking for self-motivated, well organised, and enthusiastic
individuals with a passion for community.
The post-holder will work with at-risk and vulnerable adults within the
community especially those of hard to reach backgrounds.
Social Prescribing helps to empower patients to take control of their
own health and wellbeing through sign-posting and looking at alternatives to
medicines. Social prescribing can help PCNs to strengthen community, promote
independence and reduce health inequalities.
Main duties of the job
The post
holder will work with the General Practice team to meet the needs of patients,
following policy and procedures.

Conduct
follow-up calls with patients who have missed screening appointments (including
AAA, breast, cervical, and bowel cancer screenings) as well as lung health
checks and reviews.

Work
to increase appointment attendance, particularly uptake on immunisations,
including childhood immunisations, Covid vaccines, Flu, Shingles, Pneumonia
etc.

Educate
and encourage patients on the importance of attending these appointments.

Educating patients in the community of the
importance of vaccinations through:
-Attending
family hubs
-Attending
community Sanctuary groups to support patients seeking asylum
-Attending
community centres and places of interest for vulnerable people and their
families
-Working
with learning disability agencies who support the local community and educating
both patients and carers on the importance of health checks and vaccinations.

Support
patients with specific long-term health conditions, which will include some
Saturday working to help facilitate attendance at the practice for basic health
checks (e.g. BP, height, weight).

Keep
accurate records of the patient numbers that have been reached and regularly
update Managing Partner with the statistics.
You will be supporting personal choices for patients to help improve
their well-being and referring patients to comprehensive social care services
where appropriate.

within the practice and externally to refer patients to a wide variety of
non-clinical programmes and services using a more holistic approach to health
and wellbeing. It allows patients to design their own personalised solutions to
their health (their social prescription) by addressing and understanding social
deprivation and needs. It seeks to address patient needs across several areas
such as physical activity, diet and nutrition, mental health and social support.
It is often referred to as a more than medicine approach to health and
wellbeing as it is considered that many long-term conditions can be helped by
lifestyle change alone.
As a busy GP Practice with over 27,000 patients, you can expect
fast-paced, full clinics and so excellent time management skills will be
essential. You will be expected to manage your own time.
You can expect to see patients who:

The post-holder will support patients to access health care, community
groups and statutory services for practical and emotional support. You will
work under direction of the Team Lead and with the wider team to support
patients with their health choices, strengthen community engagement and
personal resilience, reduce health and wellbeing inequalities by addressing the
wider factors of health.
The post holder will act within professional boundaries and seek advice from
colleagues when needed.
The Practice has a commitment to excellent patient care, quality
improvement and teaching. It is expected that all members of our clinical team
are actively committed to supporting these aims and participate in relevant
activities when appropriate to do so.
Please note, this role will include working Saturdays.
