Job summary

To deliver evidence-based practice for patients presenting with long-term conditions, and the provision of preventative health care to the practice population. You will work with the practice team to meet the needs of patients.

Main duties of the job

The post holder is responsible for ensuring the delivery of safe and effective nursing care to the whole practice population which will include :

  • Management of long term conditions
  • Deliver appropriate treatment, screening and advice within the professional competence of the post-holder to the practice population
  • Develop and manage care programmes for individual patients within specific groups
  • Support and deliver Cervical Screening
  • Deliver immunisation programmes
  • Health Promotion
  • Participate in Quality Outcome Framework (QOF) and Extended Primary Care Service objectives
  • To work within the NMC Code of Conduct

We are a friendly and motivated team of 8 GP Partners, 2 salaried GPs as well as an Advanced Nurse Practitioner, Clinical Pharmacist, Pharmacy Technician, 4 Practice Nurses, 2 Health Care Assistants and an excellent administration support team. We strive to improve access, streamline workload and maintain a healthy work life balance. Training F2 and ST1 doctors. GMS practice with over 13,000 patients. CQC inspection rated as good overall.

Job description

Job responsibilities

Assist in and perform routine tasks related to patient care as directed by senior nursing staff and GPs

Chronic Disease Management

Cytology

Travel clinic vaccination and advice

Asthma annual reviews/peak flow readings

Venepuncture

Hypertension management and reviews

Routine child and adult immunisations

Chaperoning and assisting patients where appropriate who are being examined by another clinician

Requesting pathology tests, for example urine culture, swabs

Interest in Diabetes, Coronary Heart Disease and other heart conditions

Interest in contraception

Following agreed clinical protocols with referral GPs as appropriate

Maintaining and checking own equipment is fit for use

Assist nursing team in stock rotation as required

Take part in fridge monitoring as required

Participation in administrative systems in the Practice

At all times there will be a need to maintain accurate records

A duty to advise GP/Practice Manager of potential problems or errors within a range of assigned tasks

Attend and participate in any Practice meetings when required

On occasion there may be a requirement to undertake home visits

Infection control

Completing EDIs and tasks in a timely fashion

Attend training required and maintain registration and re-validation
