We are looking for a passionate and experienced Practice Nurse who is confident in delivering core nursing duties, including childhood immunisations, cervical screening, and managing patients with complex long-term conditions.
Our clinical team is backed by a highly skilled reception and administration team, ensuring you can focus on what matters most delivering excellent patient care.
Main duties of the job
No two days are ever the same! As a Practice Nurse, you will:
- Manage patients with long-term conditions, providing health promotion and self-management advice.
- Deliver high-quality care, including wound management, childhood vaccinations, and cervical screening.
- Build strong relationships with patients to fully engage them in their health journey.
- Work collaboratively within the multidisciplinary team across the PCN.
- Contribute to quality improvement initiatives, ensuring compliance with CQC standards and best practices.

We are part of the Reading Holybrook Primary Care Network (PCN), a large multi-disciplined team operating across three sites in Reading (in an integrated way). Our team includes GPs, Practice Nurses, HCAs, Nurse Practitioners, Paramedics, and Clinical Pharmacists, all working together to deliver exceptional care to our patients.
Reading Holybrook PCNis a vibrant and innovative healthcare provider, serving a diverse community of about 36,000 patients across three sites. As a practice nurse at Broad Street Health Centre, youll be part of an established treatment room team of nurses and HCAs working alongside a supportive and dedicated clinical team.
As highlighted, this role will primarily be based at Broad Street Health Centre but will require cross-site working at our sister practices - Abbey Medical Centre and Milman and Kennet Surgery.

- Registered Nurse (RN) with current NMC registration.
- Proficient in nursing skills such as immunisations, wound care, chronic disease management, and general health checks.
- Active smear taker with evidence of up-to-date training.
- Experience in the UK childhood immunisation programme.
- Strong communication skills and ability to work collaboratively in a multidisciplinary team.
- Excellent organisational skills and ability to manage time effectively in a busy environment.
- Enthusiasm for continuous learning and professional development.
- Previous experience in primary care or community nursing settings.
Desirable
- Use of EMIS clinical system.
- Experience in supervision and development of junior staff /students and HCAs
- Leadership qualification
- Enhanced qualifications in long term condition management
- Enhanced wound care management (Leg Ulcers and Tissue Viability)
