Job summary
Cerne Abbas Surgery is seeking an Integrated Team Nurse to join a proactive team dedicated to providing high quality nursing care to patients both in the practice and in their own homes.
All of our nurses work in both the practice and community so skills in at least one of these settings will be an advantage, as would enhanced care skills in one or more long term condition, although training will be provided for the right candidate to reach the skill level required.
The nurse team work between 8.30 and and 7pm weekdays, with weekend working on a rota basis.
The role is advertised as full or part time, with a minimum requirement for applicants to work 22.5 hours per week: please confirm the number of hours you are applying for in your application.

About us
Cerne Abbas Surgery is an innovative, three partner rural
dispensing General Practice near Dorchester serving 4,200 patients. We operate
an integrated nursing team (INT), delivering both practice and community
nursing care to all of our patients. We topped the list for Dorset in the
National Patient satisfaction survey for 2023 scoring 98% and we are rated Good
by the Care Quality Commission.
We are a dispensing practice and provide medication to most of
our patients, dispensing over 9,000 items a month.
The surgery is an integral part of Mid Dorset PCN, a group of 8
local practices working closely to provide services to patients. The practice's
Business Manager also manages the PCN and one of the GP partners is the PCN's
Clinical Director.
The practice supports the development of it's staff to ensure
job satisfaction and a high quality of patient care.

The following are the core responsibilities of a Practice
Nurse. There may be a requirement to carry out other tasks which will be
dependent upon factors such as workload and staffing levels:
- To support
the Infection Prevention Control (IPC) lead and to work to the IPC Policy providing audits, training, support and
evidence of compliance to the Business Manager
- To support the care management programme for
the frail and housebound patients registered at the practice
- To
assess the needs of patients ensuring the provision of healthcare is
appropriate, incorporating evidence-based practice
- To
develop, implement and embed health promotion and wellbeing programmes
- To
implement and evaluate individual treatment plans for chronic disease
patients that promote health and wellbeing
- To
identify, manage and support patients at risk of developing long-term
conditions, preventing adverse effects to patients health
- To provide routine nursing care to patients in accordance with
clinical based evidence, NICE and the NSF
- To diagnose and manage acute and chronic conditions
- To
review patient medications to enhance compliance
- To
provide guidance to patients in the use of prescribed and over-the-counter
medications regarding side effects and dosages

maintain accurate clinical records in conjunction with extant legislation
