A
new, exciting and innovative Occupational Therapy role has been created by Mid Dorset PCN to support
the care of our patients, specifically those in our more rural practices where
access to core services that it is sometimes difficult to access.
The successful candidate will bring unique skills to deliver
Occupational Therapy to maximise health, wellbeing
and independence.The post holder will
work closely with staff at three of our GP surgeries who have integrated nurse
teams delivering both general practice nursing and community nursing.
Mid
Dorset PCN has a population of around 49,000 patients, with around 13,000 being
registered at our three rural practices.
We believe this role will be instrumental in achieving the best outcomes
for the people living in our communities. We welcome applications for a B7 role
but are also able to offer a developmental role initially at B6 level to map
into B7 post in 12 or 18 months for the right candidate.

Main duties of the job
This post requires an Occupational Therapist who is highly specialised
and can work at an advanced level of practice within the PCN, utilising in-depth knowledge, clinical skills and experience to provide intervention for the presenting patient
from initial clinical assessment, diagnosis and treatment, to the evaluation of
their care.
You will demonstrate safe clinical decision-making and expert care for
people within the PCN alongside having a strong understanding and use of risk
assessments.
You will work collaboratively with colleagues in the PCN Integrated
Nurse Teams to meet the needs of individuals and carers, support the delivery
of new policy and procedures, and provide occupational therapy leadership as
required.
The post holder will be required to develop,
implement and evaluate a seamless occupational therapy support service across
the three rural practices in the PCN, working with community and secondary care and other PCN post holders, such as clinical pharmacists, first contact physiotherapists and paramedics.
The post holder will work across the three GP practices with Integrated Nurse Teams, liaising also with the GPs,social prescribers and care co-ordinators who work as part of the teams, to provide the best care for our patients.
About us
Mid Dorset Primary Care Network is a PCN made up of nine GP practices in the Dorchester area of Dorset. The practices have a history of collaborative working and provide care for around 49,000 patients.
Three of the GP practices in the PCN have Integrated Nurse teams who deliver both practice and community nursing to the practice patients. The teams pride themselves on their ability to provide continuity of care to their patients.
The three teams are based at individual practices and are hard working, dedicated and have a problem-solving attitude. You will be employed by one of the practices but will work across all three and carry out home visits where appropriate.You will be working with dynamic, friendly, fun and proactive teams leading change in healthcare.

wheelchair access, technological needs, and ergonomic support.

families or carers, on specialist equipment and organisations that can help
with daily activities.

Help patients to adapt to and
manage their physical and mental health long-term conditions, through the
teaching of coping strategies.

Develop, implement and evaluate a
