Job summary

A new, exciting and innovative Occupational Therapy role has been created by Mid Dorset PCN to support the care of our patients, specifically those in our more rural practices where access to core services that it is sometimes difficult to access.

The successful candidate will bring unique skills to deliver Occupational Therapy to maximise health, wellbeing and independence.The post holder will work closely with staff at three of our GP surgeries who have integrated nurse teams delivering both general practice nursing and community nursing.

Mid Dorset PCN has a population of around 49,000 patients, with around 13,000 being registered at our three rural practices.

We believe this role will be instrumental in achieving the best outcomes for the people living in our communities. We welcome applications for a B7 role but are also able to offer a developmental role initially at B6 level to map into B7 post in 12 or 18 months for the right candidate.

Main duties of the job

This post requires an Occupational Therapist who is highly specialised and can work at an advanced level of practice within the PCN, utilising in-depth knowledge, clinical skills and experience to provide intervention for the presenting patient from initial clinical assessment, diagnosis and treatment, to the evaluation of their care.

You will demonstrate safe clinical decision-making and expert care for people within the PCN alongside having a strong understanding and use of risk assessments.

You will work collaboratively with colleagues in the PCN Integrated Nurse Teams to meet the needs of individuals and carers, support the delivery of new policy and procedures, and provide occupational therapy leadership as required.

The post holder will be required to develop, implement and evaluate a seamless occupational therapy support service across the three rural practices in the PCN, working with community and secondary care and other PCN post holders, such as clinical pharmacists, first contact physiotherapists and paramedics.

The post holder will work across the three GP practices with Integrated Nurse Teams, liaising also with the GPs,social prescribers and care co-ordinators who work as part of the teams, to provide the best care for our patients.

About us

Mid Dorset Primary Care Network is a PCN made up of nine GP practices in the Dorchester area of Dorset. The practices have a history of collaborative working and provide care for around 49,000 patients.

Three of the GP practices in the PCN have Integrated Nurse teams who deliver both practice and community nursing to the practice patients. The teams pride themselves on their ability to provide continuity of care to their patients.

The three teams are based at individual practices and are hard working, dedicated and have a problem-solving attitude. You will be employed by one of the practices but will work across all three and carry out home visits where appropriate.You will be working with dynamic, friendly, fun and proactive teams leading change in healthcare.

a. Assess, plan, implement, and evaluate treatment plans, with an aim to increase patients productivity and self-care.

b. Work with patients through a shared-decision making approach to plan realistic, outcomes-focused goals.

c. Undertake both verbal and non-verbal communication methods to address the needs of patients that have communication difficulties.

d. Work in partnership with multi-disciplinary team colleagues both inside and outside of the practice teams in treatment planning to aid rehabilitation. This includes working with other PCN ARRS roles including First contact Physiotherapist, social prescribers and care coordinators.

e. Where appropriate, support the development of discharge and contingency plans with relevant professionals to arrange on-going care in residential, care home, hospital, and community settings.

f. Periodically review, evaluate and change rehabilitation programmes to rebuild lost skills and restore confidence.

g. As required, advise on home, school, and workplace environmental alterations, such as adjustments for wheelchair access, technological needs, and ergonomic support.

h. Advise patients, and their families or carers, on specialist equipment and organisations that can help with daily activities.

i. Help patients to adapt to and manage their physical and mental health long-term conditions, through the teaching of coping strategies.

j. Develop, implement and evaluate a seamless occupational therapy support service across the PCN, working with community and secondary care where appropriate, and aimed at continuously improving standards of patient care and wider multi-disciplinary team working.

k. Provide education and specialist expertise to PCN staff, raising awareness of good practice occupational therapy techniques.

l. Ensure delivery of best practice in clinical practice, caseload management, education, research, and audit, to achieve corporate PCN and local population objectives.

Operational Responsibilities include:

a. Attend in-house governance, educational and staff meetings as appropriate.

b. Understand own role and scope in the organisation and identify how this may develop over time.

c. Work as an effective and responsible team member, supporting others and exploring the mechanisms to develop new ways of working.

d. Accept delegation from senior staff, prioritise own workload and ensure effective time management strategies are embedded in own practice.

e. Contribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the teams performance.

f. To manage changing workloads that includes frequent interruptions and unpredictable work patterns including an element of unsociable hours as and when required.

  • BSc in or pre-reg MSc in Occupational Therapy under a training programme approved by the Royal College of Occupational Therapists.
  • A registered member of the Health and Care Professionals Council and the British Association of Occupational Therapists.

  • Experience of research and audit.
