Job summary

To address the ever-increasing demand for health care services and to better support individuals within their own communities, there is wide recognition of the need to embrace new ways of working and digital healthcare technologies to ensure all patients receive the right care, in the right place, at the right time. The role of the Digital Care Coordinator will be to support patients and healthcare professions to maximise the potential presented by new digital healthcare technology and the associated new ways of working, these include: Healthcare Apps Wearables and remote monitoring technology Data and intelligence e.g. Population Health data Digital care coordination is a mechanism for delivering 21st Century care and connecting people with both medical and non-medical sources of support, empowering people to better self-manage their conditions and/or work collaboratively with healthcare professionals to manage their conditions.

Main duties of the job

Be an ambassador for Digital with patients and colleagues in equal measure Run regular proactive reports to identify a cohort of patients who may benefit from new services/ new ways of delivery services Act as the first point of contact for digital queries Demonstrate and educate colleagues about how to optimally use digital technologies and data and intelligence Troubleshoot for colleagues and patients Take the lead in the deployment of new technologies and education of patients Support the education of colleagues in line with implementation plans Support Public Health campaigns, working with the PCN Management team to achieve this To support the PCN in successful delivery and implementation of services

About us

Mid Dorset Primary Care Network is a PCN made up of nine GP practices in the Dorchester area of Dorset. The practices have a history of collaborative working and provide care for around 49,000 patients. As we grow, we are looking to recruit a full time PCN Digital Care Co-ordinator, working over 5 days to support the development of digital in our Primary Care Network. The successful applicant will be employed by and based at Cerne Abbas Surgery, working with the nine general practices at scale to successfully implement primary care initiatives. The post holder will be part of the PCN Management Team, reporting to the PCN Business Manager. Although based at Cerne Abbas, you will at times be required to travel to work across the PCN. There will be occasional evening working and potential for some home working at times.

Qualifications

Essential

  • Educated to NVQ 3 level or equivalent in a relevant subject with significant equivalent previous proven experience.

Experience

Essential

  • Demonstrable experience in dealing with people, and sensitive and confidential information.
  • Significant skills and knowledge of Microsoft IT systems and other software programmes.
  • Understanding of Confidentiality and Data Protection.

Desirable

  • NHS experience, preferably in primary care.
  • Knowledge of NHS structures, primary care contractual and commissioning processes.

Skills

  • Clear communicator with excellent writing, data entry and presentation skills: capable of constructing and delivering clear information/ instructions. High level of interpersonal and communication skills, assertiveness and diplomacy.
  • Ability to work under pressure in a busy working environment and able to multi-task.

  • Strong technology skills. IT literate and proficient in the advanced use of technology, including Microsoft and other packages.
  • The ability to collate information and carry out data analysis.
