Job summary

A new, exciting and innovative role has been created by Mid Dorset PCN to support improving health and wellbeing in children and young people through creative and joint working to promote healthy lifestyles. This role will involve working with young people and their families in the Dorchester area to deliver our Five Ways to Wellbeing programme, encouraging young people take responsibility for their own wellbeing focusing on weight, physical activity, mental health and diet.

Main duties of the job

Data shows that in Mid Dorset PCN 30% of children in reception class and 41% in year 6 are overweight or obese (2018-2019), yet we know that Young people with a good level of wellbeing are more likely to perform well at school and be physically healthy (DoH 2011). The programme has been initiated in our secondary school and middle schools, but now needs a dedicated lead to build on this early progress and galvanise the programme across other age groups and organisations. About us

Mid Dorset Primary Care Network is a PCN made up of nine GP practices in the Dorchester area of Dorset. The practices have a history of collaborative working and provide care for around 49,000 patients. As we grow, we are looking to recruit a full time Children & Young People's Wellbeing Co-ordinator, working over 5 days to support the development of our Primary Care Network. The successful applicant will be employed by and based at Cerne Abbas Surgery, working with the nine general practices at scale to successfully implement the requirements of primary care initiatives. The post holder will be part of the PCN Management Team, reporting to the PCN Business Manager. Although based at Cerne Abbas, you will at times be required to travel to work across the PCN. There will be occasional evening working and potential for some home working at times.

This will include: Taking ownership of and completing appropriate follow-up actions for projects. Developing or using existing reporting and audit tools to assist with monitoring, updating and reporting. Preparing reports, project plans, documentation and other communications in relation to the programme of work. Identify and develop relationships with stakeholders and other support teams. Use data to identify areas of specific need and work with partners to deliver solutions. Carry out audit and review to continually evaluate the programme. Develop partnership working and positive relationships between schools and GP surgeries that actively promote public health messaging. Further develop the stakeholder group that has been established, to carry this work on to meet the health needs of this population and support the partners working with children and young people. Embed joint initiatives, such as SugarSmart into schools and GP surgeries. Engage with Public Health programmes relating to child & adolescent health and wellbeing such as food poverty and the Holiday Activity Food Programme.   • Ability to work under pressure in a busy working environment and able to multi-task.
  • Effective team worker with the ability to motivate and engage others.
  • The ability to collate information and carry out data analysis.
  • Strong technology skills. IT literate and proficient in the advanced use of technology, including Microsoft and other packages.
  • Ability to use initiative, act, make decisions and solve problems independently, and know when to refer upwards.
  • Ability to provide and receive sensitive information in a confidential manner, resolve queries and conflict using tact and diplomacy.
