Job summary
Streamside Surgery is seeking an enthusiastic and skilled Chronic Disease Practitioner to join our friendly, supportive primary care team. The post holder will lead on the management of patients with long term conditions, including diabetes - hypertension - asthma and COPD - delivering safe - evidence based, and patient centred care.
The role is suitable for either a Registered Nurse or Pharmacist with appropriate experience and an Independent Prescribing qualification
Main duties of the job
Deliver structured chronic disease reviews in accordance with national and local guidance
Manage patients with diabetes and respiratory disease, including optimisation of medicines
Formulate -review and amend personalised care and treatment plans
Support patients with selfmanagement -lifestyle adviceand prevention strategies
Work collaboratively within the multidisciplinary primary care team
Maintain accurate, timely, and professional clinical records
