Job summary

We are looking to welcome an enthusiastic chronic disease practitioner to manage patients with chronic diseases (diabetes, hypertension, asthma and COPD).

We are open to people from a variety of different professional backgrounds including pharmacists and nurses.

If you are a nurse by profession, then treatment room skills such as wound care are desirable but not essential. It is essential that you have experience of providing chronic disease case to patients with diabetes and respiratory disease, and making treatment plans including changes to medication/

15 - 18 Hours per week.

Pay dependant on experience. Independent prescribing is essential.
