Job summary
An exciting new opportunity has
arisen for us to recruit a 15 hour part-time Social Prescribing Link Worker
(SPLW) to join our rapidly expanding practice team working 2 days per week.
The post holder will be an
integral part of the social prescribing team which will work across constituent
practices and the wider Primary Care Network, as well as part of broader community groups and
their multi-disciplinary teams.
These roles will provide
additional flexibility to Primary Care Networks by adding to the growing
expansion of the primary care workforce which was seen as an immediate priority
in the People Plan 2020/21.

Main duties of the job
Social
prescribing is a role created to assist in empowering individuals to take
control of their own health and wellbeing, by focusing on the individual and
taking an holistic approach by connecting patients to community groups and
statutory services for practical and emotional support.
You
will support existing groups to be accessible and sustainable and help people
to start new community groups, working collaboratively with all local partners. This role will therefore require a
significant level of multi-agency working.
The
role will be mainly dedicated to ensuring ease of access to services for more
complex patients throughout the healthcare system.
You
will be responsible for working with patients, their families and carers, to
improve their health and wellbeing by helping them access appropriate services.
The
role will require management and prioritisation of your own caseload, in
accordance with the needs, priorities and any urgent support required by the
individual. It is therefore vital that
you have a strong awareness and understanding of when it is appropriate or
necessary to refer people back to other health professionals/ agencies, when the
individual's needs are beyond the scope of this role.

Essential
- GCSE Grade A-C in Maths and English or skills level 2 Maths & English or equivalent
Desirable
- Relevant health and/or social care qualification or equivalent experience
- NVQ Level 3, Advanced Level or equivalent qualifications in Social Prescribing or working towards
Experience
Essential
- An understanding of the principles of Social Prescribing and its place within primary care
- Significant experience of delivering services to vulnerable individuals who may have multiple needs
- Experience of empowering individuals to make informed choices regarding their physical and emotional wellbeing
- An understanding of safeguarding for vulnerable adults and ability to implement relevant policies and procedures
- Experience of collaborative / partnership working and of building relationships across a variety of organisations
- Experience of writing reports and presenting information in an informative way to different audiences
Desirable
- Significant experience of working in partnership and collaboratively with agencies across the public and voluntary sector
- Applied knowledge of risk assessment and management
- Experience of data collection and providing monitoring information to assess the impact of services
- Training in motivational coaching and interviewing or equivalent experience
