Job summary

This is an exciting opportunity for an experienced Practice Nurse to join our friendly team. It's a part-time role (5 or 10 hours per week) with flexibility in the day/s and time/s of work and willingness to cover annual leave and sickness when required. Salary is dependant on experience.

Main duties of the job

Chronic Disease Case Management/ identification and register management for COPD, Asthma, Hypertension, Diabetes, CVD

Child and Adult Immunisations

Cervical Cytology

Health promotion and preventative education and care

Visits to housebound and care home patients

Key responsibilities

Clinical Nursing Practice

  • The Practice Nurse works independently using own initiative and is accountable for their own professional actions.
  • Administers adult and child immunisations and vaccinations in accordance with national and local programmes.
  • Is competent in anaphylaxis and resuscitation techniques.
  • Ensures safe storage, rotation and disposal of vaccines and drugs and adequate stock levels are maintained
  • Undertakes procedures to include venipuncture, glucose tolerance tests, BP management, urinalysis, peak flow, spirometry, new patient health checks, cytology and wound management

  • Provides a comprehensive family planning advice and referral

  • Undertakes collection, storage and despatch of pathological specimens includingintravenous blood samples, swabs, smears etc and ensure cleanliness ofequipment used.

  • Performs any investigatory procedures requested by the GPs

  • Recognises issues and gathers sufficient information to refer (e.g. drugs, domesticviolence, child protection, vulnerable adults, senior patients, social problems)

  • Care and Programme Management

  • Management of chronic diseases (including diabetes, asthma, COPD management, hypertension, CVD), undertakes the establishment and running of disease management clinics

  • Initiates the process of diagnosis with patients suspected to have a chronic disease, referring to other clinicians as appropriate

  • Manages programmes of care for all areas of womens health including providing and evaluating care under agreed local/national guidelines, protocols and patient group directions

  • Participates in annual influenza campaign and COVID-19 vaccination (which may include evening or weekendworking).
