Job summary
We are seeking a compassionate, well-organised, and proactiveCare Co-ordinatorto take on the role ofAdministrative Lead for Safeguarding and Palliative Carewithin our General Practice team. This pivotal role ensures that patients with complex care needsparticularly those in vulnerable situations or requiring end-of-life carereceive coordinated, timely, and effective support.
As part of the wider primary care and multidisciplinary team, the postholder will be responsible for the administrative oversight and facilitation of safeguarding and palliative care processes, ensuring robust systems are in place to support the clinical teams and maintain compliance with legal and regulatory frameworks.
Main duties of the job
Act as the main administrative contact for safeguarding and end-of-life care across the practice. Maintain accurate safeguarding registers, case records, and ensure timely information sharing with local safeguarding partners. Organise safeguarding meetings, take minutes, track actions, and support compliance with training records and CQC standards in partnership with the Safeguarding Lead GP and Practice Manager.
Maintain and update palliative care registers. Coordinate monthly Gold Standards Framework (GSF) or MDT meetingspreparing agendas, documentation, and follow-ups. Liaise with external providers such as hospices, district nurses, and community services to support the implementation of personalised care plans, including DNACPRs and ReSPECT forms.
Track referrals and facilitate communication between healthcare professionals, patients, and families. Provide general admin support to care navigation and clinical teams. Help patients and carers understand care pathways and ensure service continuity. Create and minute weekly MDT meetings, monitoring action completion. Support audits and reporting as required.
