Job summary
An exciting opportunity has arisen following the formation of the Hawthorn & Merchiston PCN in Swindon, for an exceptional leader to join two thriving practices with experienced and supportive Practice Managers to lead the NEW PCN function.
The PCN is in it's earliest stages and this role affords the applicant the opportunity to create an exceptional new team. Providing support to two great teams of Health Care professionals, treating over 26000 patients and helping create a service that we can all be proud to be a part of.
You will be responsible for the delivery and monitoring of projects and services committed to or
commissioned by the PCN. And you will provide oversight of the financial management, resourcing
and regulatory compliance of the work of the PCN.

Main duties of the job
HR
- Work closely with H&M Health Partnership to oversee the recruitment and retention of staff
employed by the PCN.
- Monitor skill-mix and deployment of
staff.Maintain up-to-date HR documentation.
- First point of contact for Practice Managers with regard to delivery of PCN projects and schemes.
- Manage contractual requirements on behalf of the PCN.
Finances
- Manage the PCN budget and seek ways to generate income.
- Monitor spend and allocation of monies received.
- Understand and report on the financial implications of contract and legislation changes
Data and Systems
- Collect baseline data, measuring the progress and assessing the effectiveness of all PCN projects
and services
Local Relationships
- Build effective relationships with the local PCNs, and other local PCN Managers.
- Liaise with and support the local Patient Participation Groups (PPGs).
- Support practice managers in various elements of management within practices as mutually
agreed.
Communication
- Recognize the importance of effective communication
within the team.
- Communicate effectively with all colleagues and network practice staff.
- Recognise peoples needs for alternative methods of communication and respond accordingly.
- Apply professional standards.
Organisational
- Attend PCN Manager & PCN Board meetings and ensure that minutes are kept.
- Convene meetings, prepare agendas and ensure distribution of minutes as necessary

Job Title Primary Care Network Manager
Responsible for the delivery and monitoring of projects and services committed to or
commissioned by the PCN. They also provide oversight of the financial management, resourcing
and regulatory compliance of the work of the PCN
The PCN covers a total population of close to 30K patients.
Hawthorn and Merchiston practices have agreed
to work together to deliver the PCN agenda as well as other projects where collaboration
would benefit both practices and their patients.
HR
- Arrange timely and safe recruitment for services and projects.
Work closely with H&M practices to oversee the recruitment and retention of staff
employed by the PCN and on Network projects.
- Co-ordinate intra-practice communications within the network
- Ensure that all PCN staff are legally and gainfully employed.
- Monitor skill-mix and deployment of
staff Ensure that PCN staff have a suitable induction and are adequately trained to fulfil their role
- Ensure that there is an effective appraisal and monitoring system for PCN employees
- Support and mentor PCN staff, both as individuals and as team members where appropriate
- Implement effective systems for the resolution of disputes and grievances including the
disciplinary system
- Keep abreast of changes in employment legislation
- Maintain up-to-date HR documentation (including job descriptions, employment contracts, DBS,
Mandatory Training, immunisation records and employment policies)
- Maintain adequate staffing levels to meet PCN commitments
