Network Pharmacist

Job summary

An exciting opportunity has arisen for a Pharmacist working within our PCN but linked to the Canford Heath Group Practice.

Poole North Primary Care Network (PCN), is a friendly, inclusive and forward thinking PCN comprising of a team of six pharmacists and four techs with a admin assistant and a "hub" office based at Creekmoor, Oakdale.

Each Pharmacist is linked to a "base" practice and although you will work with your PCN colleagues your duties will be to support the patients and clinical colleagues within the practice, driving local and national incentives, supporting the practice to achieve its potential and being a pivotal driver of change within the practice, seeing patients in clinics 50% of your time and working on projects and administration the other 50%.

One of your days will be your "hub" day where you will supervise the pharmacy technicians, answering questions and queries, supporting them in their daily work and helping guiding them in any projects they are responsible for to ensure clinical safety.

Main duties of the job

The post holder will be supported to develop their role to become a non-medical prescriber and it is a requirement of this role that you will be willing to work towards this and undertake the PCPEP course details of which can be found:

You will be supported to see patients for Heart Failure reviews, medication issues, structured medication reviews and any areas of interest which help innovate and drive change, including supporting the practice achieve it's QOF targets and supporting the PCN achieve its IIF and any local initiatives such as the Clinical Commissioning Local Improvement Plan.
