We are seeking a self-motivated, disciplined, and enthusiastic Nurse to join our Home Visiting Service on a fixed-term basis to cover maternity leave. This is an excellent chance to play a central role in delivering compassionate, patient-centred care within our multidisciplinary Primary Care Network (PCN) team.

The working pattern for this part-time role is between 22.5 and 28 hours per week.

Main duties of the job

  • Deliver high-quality, accessible, and personalised care for patients with long-term conditions.
  • Provide patient care both virtually and face-to-face in the community.
  • Work closely with GPs and other clinical teams to improve health outcomes.
  • Contribute to the ongoing development and innovation of our PCN services.

About us

About Coastal Fareham & Gosport PCN

Coastal Fareham and Gosport Primary Care Network was formed in 2019, bringing together three established independent practices:

  • Brook Lane Surgery
  • Lockswood Surgery
  • Stubbington Medical Practice

With a patient population of approximately 39,000 we serve a largely suburban area between Portsmouth and Southampton. We are equidistant from Queen Alexandra Hospital and Southampton General Hospital, allowing us to collaborate with major local healthcare providers.

Our mission is the practices within the PCN will work collaboratively, whilst maintaining their independence, to provide additional services for all our patients. We aim to empower patients to improve their own health and wellbeing.

Clinical Care

  • Provide nursing care to housebound patients and those in care homes, in line with the Enhanced Health in Care Homes (EHCH) requirements of the PCN DES.
  • Deliver compassionate, personalised care tailored to individual patient needs.
  • Assess, plan, implement, and evaluate care across the full patient pathway, working in partnership with patients, carers, and multidisciplinary teams (MDTs).
  • Carry out physical, psychological, and emotional assessments to ensure holistic care.
  • Provide physical, psychological, and therapeutic interventions as appropriate.
  • Deliver care visits as scheduled by the HVS Team Lead or care navigators, based on clinical need.
  • Support national vaccination campaigns for patients in care homes.
  • Complete and share advanced care plans, ensuring patient-centred decision making.
  • Conduct long-term condition reviews in line with the Quality Outcomes Framework (QOF).
  • Proactively identify and address patient needs to promote health and prevent deterioration.

Coordination & Collaboration

  • Work collaboratively with community care staff and partner organisations to provide joined-up, high-quality healthcare.
  • Act as a liaison between local organisations (e.g., voluntary sector, social care) to enhance the support available for patients at home.
  • Refer to and attend MDT meetings, contributing to integrated care planning and delivery.

Support for Patients & Carers

  • Assist patients in maintaining mobility and physical activity where possible.
  • Provide carers with appropriate support, including tools to identify deterioration in a patients mental or physical wellbeing.
  • Promote mental health and wellbeing for both patients and carers.

Professional Practice

  • Maintain accurate, up-to-date documentation of all care interventions.
  • Ensure compliance with all care policies, guidelines, and legal requirements.
  • Communicate effectively, verbally and in writing, including handling highly sensitive or contentious information with empathy and professionalism.
  • Use de-escalation, mediation, and resolution techniques where necessary, upholding the professional Duty of Candour.
  • Work within NHS frameworks and adhere to the Nursing and Midwifery Council (NMC) Code of Conduct to maintain the highest standards of practice.

  • Ability to identify risk and assess/manage risk when working with individuals.
