Job summary

PRACTICE NURSE

We are currently seeking a Practice Nurse to join our enthusiastic multi-disciplinary healthcare team. Delivering a high standard of medical care to the local residents of St Helens from a purpose built surgery in Lowe House Health Care Resource Centre

Our clinical team includes: 2 Full time GPs, 1 Health Care Assistant and a full complement of admin staff. We have approximately 7,300 patients

We currently use EMIS WEB clinical system

Main duties of the job

We would like to recruit a Practice Nurse competent in immunisations, travel medicine, diabetes management, cytology, with the capability to record accurate patient notes and have a knowledge and understanding of QOF - Please see job description

We are a friendly busy Practice, we have good admin support and are part of Central Primary Care Network, networking with 7 other practices within the network.

Job description

Job responsibilities

Job summary

The Practice Nurse will be responsible for several clinical areas such as infection prevention and control, health promotion, chronic disease management, health prevention, well women and well man clinics, as well as supporting the practice management team in the reviewing and delivery of clinical policy and procedure.

The Practice Nurse will be responsible for the implementation of processes for the effective management of patients with long-term conditions using evidence-based practice including care for elderly and housebound patients.

Working as a senior member of the practice multidisciplinary team, the post holder will ensure nursing services are delivered effectively to the entitled patient population.

Ormskirk House Surgery must ensure that the post holder has access to appropriate clinical supervision and an appropriate named individual in the organisation to provide general advice and support on a day-to-day basis.
