Job summary
We have an exciting opportunity for an Advanced Nurse/Clinical Practitioner to join our super GP practice in Crawley, West Sussex. We aim to ensure that the highest levels of care are delivered to all our patients, particularly in areas such as minor illnesses and acute, on the day appointments. This is a highly supportive practice with a long-standing team including GPs, Paramedic Practitioners, Advanced Nurse Practitioner, Practice Nurses, HCAs and many more Additional Roles.

Main duties of the job
To assess, diagnose, treat, refer
or signpost patients who attend surgery with undifferentiated or undiagnosed
conditions relating to minor illness.
To undertake home visits,
primarily for acute conditions, as assessed by your duty doctor, and within the
boundaries of professional qualifications.
The post holder will use advanced
clinical skills to provide education to patients, promoting self-care and
empower them to make informed choices about their treatment.
About us
Bridge Medical Centre comprises 4 GP Partners, a nursing team of 6, Paramedic Practitioners, an Advanced Nurse Practitioner and a vast supportive administrative team. The Practice is part of the Crawley Care Collaborative PCN who are each members of the Alliance for Better Care Federation.
Our clinicians work independently as well as part of a team collaborating with our own clinical and administrative team whilst also working closely with the Additional Roles from the PCN. These roles comprise Clinical Pharmacists, Dieticians, First Contact Physiotherapists, Dieticians, Care Co-ordinators and Social Prescribers.

Position: Prescribing Paramedic
Location: Bridge Medical Centre, Crawley
Reports to: Duty Doctor / Clinical Supervisor / Practice Manager
Hours: As agreed in contract
1. Clinical Duties
-
Independently assess, diagnose, and treat patients presenting with minor illnesses.

Prescribe medications appropriately in line with national guidance and within the scope of Independent Prescribing qualification.

Undertake clinical triage and manage patients through face-to-face or telephone consultations.
-
Refer patients to GPs or other members of the multidisciplinary team when outside scope of practice.
-
Provide advice on general health care, minor ailments, and preventative measures.
-
Carry out home visits for acute conditions, as appropriate and in agreement with the duty doctor.

Deliver safe, evidence-based care in line with current best practice and within recognised professional boundaries.
2. Professional and Administrative Responsibilities

Maintain accurate and up-to-date clinical records in System One.

Ensure accurate documentation of all consultations, prescriptions, and treatment plans.

Participate in the administrative functions of the practice as required.
