We are looking for a HCA to join our fast moving and positive Practice team based in Three Bridges, Crawley, West Sussex.

The role of the HCA is to support the nursing and doctor teams in delivering patient care and maintaining a safe, effective treatment environment. Responsibilities include clinical support, patient care, infection control, stock rotation, and various routine health monitoring activities. The HCA contributes to quality and efficiency by adhering to established protocols and maintaining competence in all delegated tasks. The role strictly adheres to the boundaries of practice defined within this job description, with supervision sought for any tasks outside established competencies.

Main duties of the job

DUTIES AND RESPONSIBILITIES

Patient Care

The HCA will undertake the following patient care duties, under the delegation and guidance of the clinical team:

- Conducting new patient health checks and NHS Health Checks.

- Performing venepuncture and ensuring prompt dispatch of laboratory specimens, including blood, urine, and swabs, adhering to designated collection times.

blood pressure checks, including ambulatory blood pressure monitoring (24-hour).

Setting up and conducting ECGs a

- Measuring height and weight,

Advising on general lifestyle improvements.

- Providing smoking cessation advice, CHD reviews, hypertension reviews, and general health promotion.

- Assisting with diabetic foot checks.

- Undertaking ear irrigation, INR monitoring, peak flow recording, and wound swabs.

- Administering flu, pneumonia, and B12 injections under supervision and after adequate training.

Assisting GPs with minor procedures, including joint injections, dressing changes, IUD and Implanon insertions, coil fittings, and other clinics.

NHS Health Checks

Well Person Checks.

- Conducting patient education on lifestyle improvements such as diet, exercise, smoking cessation, and preventative health care measures.

Tasks involving immunisations, such as shingles, pneumococcal, and flu vaccines, will be performed under a Patient Specific Direction with authorised supervision.

About us

Bridge Medical Centre comprises 3 GP Partners, an ACP, Paramedic Practitioner and a nursing team of 4 and a vast supportive administrative team. The Practice is part of the Crawley Care Collaborative PCN who are each members of the Alliance for Better Care Federation.

Our clinicians work independently as well as part of a team collaborating with our own clinical and administrative team whilst also working closely with the Additional Roles from the PCN. These roles comprise Clinical Pharmacists, Mental Health Practitioners and Care Co-ordinators.

To promote health and well-being within the practice population.

To work with clinical and non-clinical practice staff, other members of the wider healthcare team and NHSE/ICG in contributing to:

The achievement of QOF.

Delivering objectives set out in the NHS Action Plan.

Audits.

Meeting the standards as set out by the Care Quality Commission (CQC).

Communicate courteously and effectively with patients, relatives and careers.

Report all matters pertaining to patient care changes in condition/circumstances to a relevant member of the nursing or medical team as a matter of priority and ensure that a document record is kept.

Ensure that the clinical rooms at all times kept in a clean and orderly manner and that relevant equipment is ready for use by nursing and medical staff during clinic sessions.

Assist in clinic sessions as directed by members of the nursing and medical staff ensuring that all equipment is clean and sterilised following clinic sessions.

Ensure that the drug refrigerator is at correct temperature on a daily basis and that drugs are within their expiry dates and used in correct sequence. Order drugs and other supplies as directed ensuring that adequate levels of stock are available in the surgery.

Record fridge temperatures.

As directed by the Practice Nurses working within local policy/protocols undertake and develop skills in phlebotomy, 24 hour ABPM, ECG recording, advanced phlebotomy, spirometry/peak flow, chaperoning duties, basic wound care, ear syringing, B12/flu/pneumococcal vaccinations, Long Term Conditions annual initial assessments.

Undertake NHS health checks new patient health checks and general health checks.

Undertake a range of diagnostic procedures e.g. urinalysis, blood pressure monitoring, pregnancy tests, diabetic foot checks, (within capabilities and as required for the role).

Delivery of health promotion e.g. smoking reduction advice, diet, exercise and alcohol.

Maintain stocks control for all clinical rooms.

Support practitioners to carry out healthcare activities.

Act at all times with regard to the confidentiality and sensitivity of issues to do with patients, their care and treatment.

Record and document all patient contact.

Carry out relevant clerical and reception duties as required in relation to nursing services.

Assist in audits as required e.g. note sampling, data input and information gathering.

Attend relevant meetings as required.

Have knowledge of and act at all times within the boundaries of Health and Safety legislation and data protection acts.

Assist in the call and recall systems in relation to: chronic disease management; immunisation, cervical cytology screening.
