Job summary

Due to career progression we are recruiting for a Social Prescriber Link Worker to join our team

Social prescribing empowers people to take control of their health and wellbeing through referral to non-medical link workers who give time, focus on what matters to me and take a holistic approach, connecting people to community groups and statutory services for practical and emotional support. Link workers support existing groups to be accessible and sustainable and help people to start new community groups, working collaboratively with all local partners.

Social prescribing can help to strengthen community resilience and personal resilience, and reduces health inequalities by addressing the wider determinants of health, such as debt, poor housing and physical inactivity, by increasing peoples active involvement with their local communities. Social Prescribing particularly works for people with long-term conditions (including support for mental health), for people who are lonely or isolated, or have complex social needs which affect their wellbeing.

This role is suitable for people who are able to work in a fast paced environment alongside other PCN and general practice colleagues. The post holder will need to work flexibly with a holistic approach.

This role is not purely a signposting role.

Main duties of the job

This is an innovative role designed to work in an outcome focused way to improve peoples quality of life, health and wellbeing by recognizing that this can be affected by a range of social, economic and environmental factors. Supporting patients and their carers to achieve their personal aspirations, participate in their local and wider communities, enhance effective personal support networks, enabling individuals to maintain healthy lifestyles; and lead independent and fulfilled lives.

You will develop links with local communities and VCS to ensure delivery of integrated, patient focused care, demonstrating flexibility in the role, and a caring attitude towards patients.

You will have excellent organisational, interpersonal and communication skills together with a good working knowledge of primary care and the challenges facing patient care in the NHS.

You will work individually with patients in a variety of settings such as GP practices, community settings and home visits.

About us

This is an exciting opportunity to join an established Social Prescribing team that also encompasses Care Coordinators, Health and Wellbeing Coaches and a Mental Health Practitioner.

Bradford City PCN-5 is a network of 10 GP practices covering the geographical area of Bradford Moor, Thornbury and Eccleshill.

You will be expected to work across a number of Practices, with a set patient population, alongside both the GP and nursing staff, to meet the needs of patients by providing direct care using established guidelines.

Further ongoing support is provided through both local and countrywide governance and peer support groups. Additional professional development in line with the individuals needs will be available with shadowing, e-learning, relevant courses/conferences and higher level training opportunities being supported.

  • Receiving and actioning referrals from a wide range of agencies: GP practices, pharmacies, multi-disciplinary teams, hospital discharge teams, allied health professionals, fire service, police, social care services, housing associations, and voluntary organisations. (List not exhaustive).
  • Working with adults with mild to moderate mental health, learning disabilities, anxiety and depression. Providing personalised support to individuals, their families and carers to enable them to take control of their wellbeing live independently and improve their health outcomes. Develop trusting relationships by giving people time and focus on what matters to them. Taking a holistic approach, based on the persons priorities and the wider determinants of health.
  • Assess the patients abilities and preferences, thinking laterally and then advising and supporting them in a variety of areas/activities. Being sensitive to barriers to adopting a healthier lifestyle such as affordability, accessibility and life circumstances.
  • Encouraging choices and actions that are acceptable and achievable to patients while being aware of cultural and social considerations.
  • Support patients to recognize and change their current lifestyle and to identify how their way of life might affect their health and well-being.
  • Working alongside and collaborating with existing local partners
  • Be proactive in undertaking community development to encourage self-referrals and connecting with all local communities, particularly those communities that statutory agencies may find hard to reach.

  • Forge strong links with local VCSE organisations, community and neighbourhood level groups utilising their networks and building on what is already available to create a menu of community groups and assets.
  • Work collectively with all local partners to ensure community groups are strong and sustainable.
  • Meet people on a one to one basis, this may be in a clinic setting, community setting or making home visits where appropriate.
  • Educating non-clinical and clinical staff within the Practice on other services that are available within the community and how and when patients can access them.
  • It is vital that the Social Prescriber has a strong awareness and understanding of when it is appropriate or necessary to refer people back to other health professionals or agencies.
  • To effectively time manage a caseload of clients, and be able to effectively prioritise workload in accordance with needs, priorities and any urgent support required by clients on the caseload and to meet scheduling requirements.
