We have an exciting new opportunity for a Clinical Pharmacist.
The post holder will work as part of a multi-disciplinary team within the Primary Care Network (PCN) in a patient-facing role.
Join our group of healthcare professionals working in collaboration with many sectors, to provide specialised / proactive/ support and coordination for individuals with complex health and psycho-social care needs in the heart of Lincoln. Covering two practices, Brayford Medical Practice and Heart of Lincoln Medical Group, this is an exciting opportunity to work with a high intensity group of people to improve health inequalities and outcomes for all!
Main duties of the job
Core responsibilities of the Complex Community Care Lead Practitioner within our sites. There may be on occasion a requirement to carry out other tasks; this will be dependent on factors such as workload and staffing levels:
They will take responsibility for areas of chronic disease management within the PCN and undertake clinical medication reviews to proactively manage patients with complex polypharmacy, as well as provide primary support to practices within the PCN, with regards to prescription and medication queries.
They will help support the repeat prescription system, medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patients of the PCN practices.
They will provide clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services.
Ensure that the practices, via the PCN, integrate with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload.
The role is pivotal to improving the quality of care and operational efficiencies, so requires motivation and passion to deliver an excellent service within general practice.
The post holder will be a nonmedical prescriber.
